Well, yesterday I played against a Modern Affinity with an incomplete version of that list, just 47 cards and no sideboard and... I stomped him. We played 4 or 5 games and I won every single time, without feeling much pressure. Not even when he sideboarded. Here goes the list I used:
Chalice of Death is an underrated card. Most of the times when it hits the board it automatically transforms and can damage that same turn.
It puts a clock on the enemy of 5 damage every turn, and prevents the opponent of attacking us as much as they would like, because if he is going to take 5 damage every turn he needs at least to keep blockers for our Ascendants. Impressed with the results.
I fail to see merit in Windbrisk Heights. It is slow and rest of the deck lacks synergies with it. Also 26 lands seems bit excessive. Chalice of life is a nice touch, but I would rather have one Aetherflux Reservoir. Drop 2 Heights and you could throw in Thraben Inspector or more O-rings.

Im almost finished building it but I know I'll decrease the land number by 2 or 3. In exchange I've thought of adding some Soul Warden and/or Thraben Inspector as you say, but isn't the Thraben Inspector a little bit slow as well?

It's easy to transform on this deck and once it's transformed into Chalice of Death, it will not shrink if I'm under 30 life (unlike Serra Ascendant).
It's a great card to use with the Windbrisk Heights and puts a clock of 4 turns on the opponent.
Anyway I can see that people don't usually use the Windbrisk Heights anymore, but I don't truly understand why. The mana department is where I'm more lost to be honest.
What adjustments would you recommend me to do? Im not even speaking about sideboard.

I will add x4 Path to Exile and x4 Ghost Quarter, another Wrath of God, another Emeria, two more Flagstones and probably one Thraben Inspector.

Maybe a Hallowed Burial or even a Felidar Sovereign in exchange of the Titan?
