Mine is definitely Cliffside Market. Most of the time it's just a lot of hassle for little gain. We usually take it out of the pile when we're playing Planechase.
Lethe Lake can be frustrating in a non-Commander environment, too.
Lethe Lake is up there, though I think the worst is whichever one (Don't remember the name) that destroys all creatures when you planeswalk away from it. Someone always manages to exploit that one.
There's also one that gives extra turns, which someone used for infinite turns during an EDH game, which is more funny as hell than annoying.
Leethe Lake is pretty bad. Sea of Sand is hated around our table too. Sanctum of Serra is a good plane, we all think, as it's usually only used when someone's at critical mass, so it never feels too unfair.

Definetly not a fan of Lethe Lake. [cardNaar Isle[/card] gets pretty dumb of no one tries to planeswalk. Also, Eon Fog is good if you're lucky, but most of the time it just makes the game rediculously slow.
I never liked Krosa. Its effect always seemed rather unoriginal to me. Flavor-wise, I suppose it makes sense, but they could've done more with it.

If you suggest a card to add to a deck, suggest one to take out as well.
Probably The Eon Fog and Lethe Lake. Graveyard based decks love the card but not everyone plays a GY based deck. The Eon Fog is just boring as hell; sure it might be good for control decks to just play land go for a few turns while everyone does nothing but it's just too swingy. When someone planeshifts away from the Eon Fog everyone is overjoyed and thinking "Yay I get to play Magic again w00t!!"

I love Lethe Lake, always fills up the graveyards quick. I really dislike the Eon Fog, as others have said, just grinds games to a halt, which is bad when you add EDH to the mix.

I'd have to echo Lethe Lake and Cliffside Market. The former, especially when it's the opening plane, just... it just never ends well. My metagame is reanimator heavy, and this just makes things spiral out of control way too quickly. The latter just because it's soo... boring. Frequently, barring Necropotence shenanigans, we just plain forget it's there until someone rolls off. This isn't intentional, we just don't really pay attention to it. It's even more annoying when no one has anything and someone rolls chaos, because then we have to keep track of who had what basic land which is just annoying.
