Thanks for this! These few cards I suppose do make the deck have a better side board or overall gameplan. Red splash seems like adding bolts for removal and blasts for the counter war and blood moon for the non-red decks.
The black splash send to go on a different route of preemptive control and can get away with more noncreature spells. This splash send to have more answers in the form of boardwipes.

Yeah, that's right. That's pretty spot on.
UWR gets you more cheap removal, and Blast effects are nice because Blue is very abundant. Blood Moon is really strong too.
Remember though, Esper gives you Lingering Souls, which although aren't actually creature spells, they do give nice bodies. Black gives more board wipes (Toxic Deluge for example), and the discard IMO is great, and sideboard options provide really strong sideboard games. But Red's primary cards for splashing are innately strong in and of itself that you might not need or care for the rest of what red offers.

Has anyone tried monastery mentor in place of true-name? And why are done lists running true-name? Are there better options for it? I have read some articles that really try to push for mentor in legacy, but it seems the weakest here to me because potto pyromancer does almost the same job for one less. Yet, mentor gets around spell snare and can make larger tokens. What are people thinking?
Monastery Mentor fits better in the Miracles shell because Countertop is such a great tool to protect the Mentor with. Mentor is a relatively high end card compared to Young Pyro so you need to be able to protect him for him to be effective at all. Stoneblade decks actually don't have that many good ways to guard against cheap removal like Bolt and StP, what are you going to use, FoW?
Mentor is one of the strongest creatures that are Legacy playable to come out in the past year, but Stoneblade may not be the best shell to build him around with.
True-Name is a pretty resilient threat. Not much really compares. I mean, the deck dropped stuff like Geist of Saint Traft and extra Vendilion Clique to make room for Nemesis if that says anything.
Mentor really requires a deck to be build around it. It's the sort of card that is very strong, but doesn't just slot into just any deck.
This is the current list I have been running in my area. It has done well, especially with the lingering souls in the list. I only recently put the TNN back into the deck. I have been considering trying out Patriot blade, but just feel that it would not have the card advantage to play the long game. Between running so much removal and various counterspells, I feel like you have too many dead cards against several match ups. Anyone have more experience with the patriot blade deck that could explain what they feel is the reason to play the deck would be appreciated. I know it does well on MTGO quite often.
I play UWr, and the two big ones are REB/pyro and Blood Moon. REB is just a ridic card in legacy (cause, well, blue), and blood moon is frequently a 1-card win condition, to the extent that I actually maindeck a copy (including some.. other "interesting" more-typically-SB things). Izzet Charm is also fantastically versatile, both able to play to the board and on the stack.

UW with no splash is a great opportunity to play colorless lands like Mishra's Factory and Wasteland coupled with Stifle and Standstill, since Factory and Wasteland are the biggest arguments for only playing two colors. If you don't want to go the tempo/landstill route, there's the artifact and enchantment heavy, more controllish version with cards like Back to Basics and the Thopter Sword combo.

That's kind of a backwards way of thinking about SBing. Usually the thought process is matchups->cards, not cards->matchups. Also, SB design does depend quite a lot on maindeck. Like, my maindeck build is extremely tilted towards fair grindy games, so nearly my entire SB is devoted to unfair matchups. My SB is 1 blood moon, 2 RiP, 1 canonist, 1 e-tutor, 1 izzet staticaster, 1 SoFaI, 2 wear//tear, 3 swan songs, 3 blasts.
Anyhow, I've been testing Narset Transcendent lately and she's been absolutely fantastic. With 4 BS 4 ponder 1 top her +1 is super consistent and her -9 just ends/steals games. Overall I've also gotten really super-friendsy. I've become underwhelmed by TNN lately, as my meta is super prepared for them, so I replaced them with 1 elspeth knight errant and 1 nahiri. Also, I'm running 2 supreme verdicts which also play better with the walkers over the TNNs.
Legacy can have very different localised metas. So unless your looking to play a large tournament, a good sideboard is going to be more individualised to your meta. My suggestion is to list the matchups you expect to see and then prioritize those based which ones you struggle with the most. At that point filling up slots for each of those matchups is the go. You can even go a step further and identify the bad cards in your mainboard for each matchup to get an idea of how many slots ideally you would want in the sideboard (sometimes not feasible, but it does give you an idea of your cut/add plan). Flexible answers that cover multiple matchups are always nice to keep in mind as well.
Also, I have to agree with tuxdev that the sideboard should be considered as part of the 75, not as just 15 cards. Generally decks like this will have flex slots that you can preboard into with the more general techs. Also, as mentioned above, knowing what you'll want to cut in particular matchups is important to developing a good plan.
If you wanted to post your main and the matchups you think you need post board help with, then it might be easier to provide more specific suggestions.
This is for an unknown meta at the moment. I have debated Elspeth in the main over a bolt. If that happens, I can add some other things like sulfur elemental or izzet staticaster, which I forgot. I have the venser instead of snap for omni matches where I can just bounced their omni off of SnT - but that may only work in theory.
I think you should focus your deck more to a particular curve and pace instead of being a little bit of everything. See the choice of countermagic for example. You're putting early-mid game, tempo oriented permission together with Counterspell, which favors grindy draw-go gameplay. Spell Snare and Spell Pierce are great in board advantage lists with a higher density of creatures, whereas Counterspell and Venser are more geared towards pure control strategies (if you're going to semi-tapout every turn in the early to mid game you can't leave UU open). If you favor CMC1 permission then exploit it with more cards like Snapcaster Mage, which is great in any WUR list with Snare, StP and Bolt. If you favor more going passive then cut sorcery speed cards like the maindeck Council's Judgment and the sideboard Canonist.
Patriot Stoneblade has traditionally had difficulty in filling up the GY quickly to exploit Delve cards like the 2 Digs you're running, so I'd personally go for more Bolts and less StPs. Counterspell is also a relatvely bad card in that regard, since it's both passive and expensive, and it's most valuable in your hand than actually casting it.
With a set of Bolts and the very solid artifact removal UWR provides I don't think catch-all removal like Judgment is really neccessary. Looking at your SB I'd like to see one more Containment Priest (in place of the Cage since it turns off your Snappers too) and 2-3 Meddling Mages, while cutting down on the SoFaF and the RiP, which hurts you a lot too. Maybe consider a 1-1 split of RiP and Relic?
Sulfur Elemental is also a great card to have, although Staticaster might be a better choice if your meta has Elves.
The beauty of Patriot is that it has a wide array of posibilities depending on the playstyle you like, ranging from pure control like Thopter Sword and Miracles to UWR Delver. There are different card choices for each tier, and what you like to play is entirely your choice. Just make sure you build the deck to focus on its particular strategy and not spread out too much.
I never thought of the deck in more of a type of midrange. I would like to make it more aggressive, but if the game goes late, that is when I would utilize the counterspell. So if I make this more aggressive, I can swap venser for a snapcaster. I can remove the counterspells and council's judgement to add the rest of the bolts and another spell pierce. Is it worth moving the verdict to the side? For a main meddling mage or some body to be slightly more aggressive?
I haven't had the chance to text SoFaF, but I have seen a lot of lists use it in side. Is it outpaced in its good matches?

I run 2 digs, and the way I see it, if I cast it early-ish, it's because the game is super dynamic and we're trading heavily. If I can't cast it because nothing is really going on in the game, I feel generally advantaged and can probably just take over with some planeswalker on an empty board and don't really need to cast it. I also don't like cutting RiPs because of Dig, because when RiP sticks, it is often just winning the game nearly by itself, and thus, you don't really need to cast Dig. I don't like running anything less than the massive sledgehammer that is RiP when I've got access to it. The only time I had Relic in my list was when I was also experimenting with Trinket Mage a bit (I already Top and EE as targets anyway, relic wasn't much of a stretch).
Tristin, if you're going for a faster list with heavy bolt count, I'd make it more like delver-less delver and run young pyro, git probe. I dropped bolt from my list ages ago, as I'm much grindier and rely on my single-card "unstoppable" win conditions rather than try to ever damage race.
Bolt is secretly super underwhelming when Lava Spike mode is strictly win-more. That may or may not have been influenced by hanging out in the Miracles threads a bit too much, as it's really hard to find good discussion on UWr stoneblade when most everybody talks about esper all the time. Oh yeah, when I played against the "new miracles" 4x mentor + daze lists a couple weeks ago, I found that I was quite definitely *not* the beatdown side of the matchup. Surreal.
If you are going to just run one, Nahiri is the far superior card over Elspeth if you can actually afford the one extra mana. When people kill/counter your equipment, they start playing as if that equipment is just gone forever. I've pretty regularly blown people out by pulling them back from the yard with Nahiri. It's also *always* disgusting to +2 Nahiri with a germ-less batterskull on board.
I would immediately cut the Cage for an E-tutor. It effectively adds another copy of all your big haymaker one-card kills in blood moon, RiP, canonnist. Makes you feel like you're running a 18-20 card sideboard.
Something to try to really mindscrew your meta: sometimes maindeck blood moon. It's actually a lot less narrow than generally thought. Along with the Narset I run, I've ended up with this funny reputation of "expect the unexpected" and they have to respect that or lose. On the same vein, I've maindecked containment priest and would seriously consider maindecking meddling mages if I actually owned some (at worst they're 2/2 bears that can carry a jitte/sword, which is still fine). A big inspiration for this style of deck building is Joe Losset maindecking blast effects ("cheater" miracles).
