One of my biggest is threat assessment. Whats worse is if I realize your a threat and I stop you, the tears start flowing. Sorry I blew up your Exquisite Blood, but you have Sanguine Bond in your hand/on the stack.
Another one is if a player has infinite turns and wants to play them out. No, you ascend and we keep playing without you.
Yes, you lost last game, no need to be a bitter child and try and troll the table because you can't learn to play better. (Not really an issue in my main play group but definitely one in repeat pick up games)
Oh also bad players. I know people have different skill levels but if you are downright bad at magic and keep trying while refusing to get better I don't want to play with you/talk to you/deal you.
i don't like people who use politics as an excuse to not get swung at. sure, you can use it when you're behind, but using it when you have a giant board(for example, just went infinite with thoptersword or something) and trying to make me attack the guy with the most Card Advantage doesn't work well with me.
EDH is the most fun when everyone at the table is on the same page competitiveness-wise.
This is true in any game.
Neither player has fun playing chess when one guy curbstomps the other all day long. League of Legends attempts to pair teams of equal skill against each other. The dude playing a "Vintage" EDH deck doesn't have fun annihilating every game he plays with his deck.
Your tuned Azami deck would be a lot more fun if everyone else at the table were on the same level and you had to fight to actually win. Nothing about its power level makes it not fun. It's the relative power.

My pet peeve when playing is when someone has a super competitive deck and the seem to think its fun to mop the asphalt with your face. That aside I don't have the income to go buy a $500 deck so mine are usually pretty casual. I understand that people have fun playing with really cut-throat decks but when you face someone who is playing a casual deck... no not cool. that's my opinion though.

Double standards, or "it's alright if I do it" mentality.
For example, if you tell me before the game that you don't accept land destruction and won't play with it, don't destroy my mana. I have had games where, after insisting that I wasn't allowed to play land destruction, I had my lands stolen by Gilt-Leaf Archdruid or had all of my mana destroyed by Fracturing Gust. They insist that it wasn't "technically" land destruction, but it leads to the exact same gamestate and has the exact same problems associated with it.
Similarly, "no infinites" means to me that you cannot do something unbounded. If you go on to recur something indefinitely, you have broken your own rule.
I don't have a problem with self imposed restrictions on gameplay, but be consistent.
I'm gonna have to go with people making ridiculously bad plays.
It's a 3-player game. Let's say I'm player 1, and player 2 and player 3 are the other players in turn order.
I was clearly winning with my mimeoplasm deck. I had like 6 fatties in play, and a full hand from my JIN GITAXIAS!
Player 2 starts his turn. He drops liliana of the veil and makes me sac a creature (doesn't help much, but the best he could do).
Player 3 goes, and SWINGS AT LILIANA WITH HIS SKULLBRIAR.
If he was just joking around in a conceding fashion, I would have been fine with it, but he wasn't. He didn't have an answer to my board, but why the hell would you attack your teammate? (basically archenemy at this point).

for example, you have your entire horde of fatties, you should see a wrath effect coming. if you didn't and complain about it for the rest of the game, then too bad
Everyone's already said it, incorrect threat assessment. Swinging out at me because I won last game is not a reason. The thing that really bothers me is the defensive justification, "had nothing better to do", "just did it for the lolz", "I don't really care if I win". That's a real problem.
Proxies are not okay in tournaments or without asking. I have a few proxies, but I print them out in full color and I will always ask if it's okay before playing. My main playgroup is fine with them because they are all competitive players and enjoy the challenge. Like people have said, it's not fun unless everyone is on the same level. Sheets of paper with pencil scribbled on them are really bothersome though.
Lastly, people who get mad about "degenerate" decks. Combos and land destruction are valid strategies. They are legal to play, and if you refuse to allow cards just because they're good, your meta will be stale and you'll never improve. Same goes for tribal decks. If you want to adhere to some arbitrary theme, that doesn't mean you should prevent yourself from playing cards that would go very well in the deck just because they aren't in the theme.
My preference is in a tournament to pick the strongest players and then gradually weaken them, if there's an infinite combo or a swift win deck I'll brutalize those decks readily first. I'll keep my one shot kill cards in the gun until I absolutely need to use them, because then the problem becomes whenever you enter final configuration you become the end game scenario yourself. But I typically hate having to rely on a glass canon solution to certain decks, as I prefer tempo decks that gradually weaken others. This is also why I typically hate it when people play any sort of mana flare effect because it allows people to combo out more readily as compared to more temp based decks that gain advantages incrementally.

Oh god yes. This one just really makes me angry. It's ridiculously immature, and it typically makes me just leave after a game or two.

Finally in my Kaho, Minamo Historian deck the are people who get upset when I tutor for answers that I need with Kaho. That's kind of the point of the deck

My friend was doing some trading at our shop and I was wandering about the playroom. Started watching a four-man EDH pod where one guy turned on Kikimite around turn eight or nine and swung to victory. The clown next to him scoffs, proceeds to untap, and tells the winner to get lost.
What do you mean, asks Kikimite?
"We need to decide who the ACTUAL winner is since you went and ruined the game."
But, I won, I beat you.
"The way you did doesn't count. Now get lost so we can finish the game."
Kikimite looks stunned and aghast. Then he proceeds to collect his cards, his dice, his tokens, picks up his soda, and dumps it out on the clown's deck.
Game over, man. Game over.
The clown then proceeded to call Kikimite a slur for homosexuals. Which caused another individual who heard him to promptly crack him in the face. And Kikimite walked away, never to be seen again.
Other annoyances would have to be those with preconceived notions they expect others to adhere to without discussion, people who apparently don't read or pay attention to things they've been asked to, and people that chew gum while playing.

Mine is when someone knows that their buddy is playing some sort of infinate/degenerate combo and they precede to make it their job to protect their buddy by preventing anyone else from stopping the combo or killing the combo player. Wile not even really trying to win their self just trying to make sure the combo player always wins. I'm fine with helping another player out. For example if I were to kill a creature attacking you or pump a blocker of yours so it lives or trades.

Targeting the winner of the last game doesn't bother me because it shows they were a threat. Unless they changed decks. I've had games where I roflstomped so I switched to a less powerful deck to make it more fun for the group and promptly get ejected ten turns before anyone else.
I also don't like when people assume that other players assume that somebody is not running something because it didn't come up in a game or two.

I have the same issue, but I also have an issue with players who are content to win on a misplay. Sometimes board states get confusing and a player that refuses to let another player quickly rescind a tutor while there's a Stranglehold on the battlefield irritates me a ton. Not a turn later, "uh let me go back and counter that", but a "yes I win if that resolves", so let me counter it two seconds later is ok in my book.
I also have issues with people who get upset when you attack them. If you have no blockers and I can safely get in for damage against you, I am going to do it. That is not mean, it's just Magic.
What drives me nuts most in EDH is the variance in metagame and power level. It's incredibly frustrating to put a new deck together and then realize that you can't play it at the table you're at without getting curbstomped or becoming the archenemy.
