You have a lot of the staples in the format already, which is good! My only other comment about the list is that you certainly have a LOT of sweeper effects, you may be able to cut back on some as having a few in hand isn't always the best.
To answer some of your questions:
1. the number of lands you should be running is more of a guideline then an exact number. Usually anywhere from 36-40, depending on your curve and other mana acceleration cards you have.
2. The curve seems a just a little high, but you do have some ramp.
3. It is very hard to make a deck that works just as well for 1v1 and for 6v6... for 1v1 you dont need as many sweepers as you do, but for 6v6 youll probably want as many as possible, so it all depends on what are playing more often.

On to the list
I used to play a similar deck, led by Tajic, Blade of the Legion. As you are playing Avacyn, you will get some hate before the game begins, as many people think she can be oppressive and very difficult to answer. However, she does cost a lot so you have to make sure you can ramp to her fast enough before other people can find their answers.
Astral Cornucopia and Everflowing Chalice aren't the best for ramping, as they require significant mana ahead of time for it to work. I would suggest more things like Marble Diamond, Knight of the White Orchid, Land Tax, Solemn Simulacrum, Caged sun, Pearl Medallion, and Pristine Talisman, to name a few.
