I believe in question 13, the one about niv-mizzet as a commander, you forgot the fact that tainted strike gives +1/+0.
It was changed from "whammer + vigilance" to Batterskull, and the numbers weren't updated - it's fixed now. Thanks for catching it, but emailing us tends to get it fixed faster than posting.

It's still lethal damage, 4+4+1 = 9 poison, this is then dealt after the 1 poison is dealt to them from the niv's trigger resolving.

Regarding the 2HG + Exsanguinate question, I believe E is also a valid answer.
I lurve 2HG. Especially when each team has 18 packs...
(Most of the joke answers are valid depending on your perception! Very rarely are they the only valid answer and actually not a joke at all, only silly-sounding...)

I'm a big fan of the special quiz columns - I quiz myself on the regular ones too. I don't really expect myself to get 2HG or 'player-leaving-the-game' questions right, because those never really come up in standard, which is the only opportunity i really have for judging.
However, I recall in the past 2HG recognized each player having a life total equal to half of the shared life. Is that a relic of the past, or is that still pertinent for things like Zendikar vampires and their '10 life or less' conditions?
Q: In what order do things happen when a spell resolves?
A: The order is...
A: Someone gets priority.
B: Put the spell into its owner's graveyard if it's an instant or sorcery.
C: Put anything that triggered on the stack.
D: Do what it says on the spell or put the card onto the battlefield.
E: Check state-based actions.
I would answer D-B-E-C-E-A since you explicitly point out that "triggered on the stack" option
Maybe you should remove "put anything that triggered on the stack." and make a note that "nothing triggers during spell resolve or state-based actions check"

I think the problem is that the question is trying to be as detailed as possible while being as brief as possible. You're wrong about nothing triggering during spell resolution, though:
116.2a Triggered abilities can trigger at any time, including while a spell is being cast, an ability is being activated, or a spell or ability is resolving. (See rule 603, "Handling Triggered Abilities.") However, nothing actually happens at the time an ability triggers. Each time a player would receive priority, each ability that has triggered but hasn't yet been put on the stack is put on the stack. See rule 116.5.
Also, both E and C can happen multiple times, but don't have to. Once both don't do anything, then A happens.
116.5. Each time a player would get priority, the game first performs all applicable state-based actions as a single event (see rule 704, "State-Based Actions"), then repeats this process until no state-based actions are performed. Then triggered abilities are put on the stack (see rule 603, "Handling Triggered Abilities"). These steps repeat in order until no further state-based actions are performed and no abilities trigger. Then the player who would have received priority does so.

Nobody actually controls replacement effects. However, when multiple replacement (or prevention) effects want to modify how an event affects a player, the player that would be affected by the event chooses which replacement effect to apply first. In this case, that's the opponent.
also, are the forced mulligan for extra cards drawn random?
It's not a forced mulligan, but yes, the two cards that are shuffled away are chosen at random. (We edited the article to make that clearer.)

13.75; a couple of the ones that I missed are things that I like to think I'd have gotten if I were a bit less sleepy, but the Mindlock Orb one and the mulligan one I legitimately didn't know, and the Liliana and Furnace of Rath ones I tend to forget the exact mechanics of even on a good day.
I guess I feel I could've gotten 15.75, so still far from perfect, but good enough for dinner at least.
