Not sure if I have just had a string of bad luck lately or what. I have had 3 separate tcgplayer orders cancelled in the last 2 weeks and refunds given due to "inventory issues." Two were from big time retailers and one from a smaller seller. It's not like the cards I bought were in the middle of a buyout either.
It's been happening to me more often in recent weeks, too (3-4 times in the last month) -- and my wants are all over the place, so it's not like I am ordering the new "Standard hotness" or anything like that.
My bigger problem has been that, for the life of me, I cannot get a single store to send me the correct version of certain cards -- in particular, I have had 7-8 stores send me Anthology versions of original Duel Deck/Commander cards in the last month alone, making it seemingly impossible to put together matching playsets of the versions I want. I'll probably just suck it up and go pay little more at SCG or something, but it blows my mind that no one on TCGplayer is really paying attention to what versions of cards they have in their inventory.

I hear you. I keep getting pack foils when I order prerelease foils, and vice versa. Usually people are quick to refund when I tell them that it's wrong, but yeah, it's annoying.

As a relevant update, I recently ordered 2x copies of the same card I have (unsuccessfully) been trying to get a playset of for the last month, and they hit me with the double whammy: refunded me for 1x copy (due to "inventory issues") and sent me the wrong version of the other 1x copy (even after I reached out to them ahead of time to confirm they had the right version). As KnickM said, I am usually able to get a quick refund, but it's time consuming and mildly irritating.
As the owner of a TCGPlayer store, I can promise you we all aren't alike. =) But for some stores it is mostly honest mistakes - they just aren't keeping track of exact sets very well. Note you can immediately reach out to a seller after a purchase, and the store front makes it really hard to not check messages before filling orders, so if you reach out to make sure they confirm exact set, you can hopefully avoid some refund/return delays.
On inventory issues, it again sometimes happens when a retail store lists the same card online, and if they aren't matching up online inventory with retail sales diligently, this can happen - they discover they are missing a card only when someone buys it from their online store.
Look for ratings and feedback on sellers - this will spot sellers than are closely monitoring their online inventory, card conditions, shipping cards on time, etc.
Well, yes, I know all of these aspects about trying to find reputable sellers -- I am an experienced buyer (and seller) who used to deal with larger volumes myself. I was just amazed/slightly amused that all of these issues were suddenly happening over the last 1-2 months, involving sellers both large and small.

Oh, I'm definitely not blaming all TCGp sellers, and honestly with the volume that I've been ordering lately (I friggin' LOVE Dominaria), it would probably be more surprising if there were no mistakes.
