Well. The first part of this is wrong; counters can exist on any player or any game object:
121.1. A counter is a marker placed on an object or player that modifies its characteristics and/or interacts with a rule, ability, or effect. Counters are not objects and have no characteristics. Notably, a counter is not a token, and a token is not a counter. Counters with the same name or description are interchangeable.
The second part of that answer is right, though; when the creature is put into a graveyard, it becomes a new game object with no memory of or relation to its previous existence; from the perspective of the game, it's no longer the same thing that was on the battlefield and had level counters on it, and doesn't remember being the thing that was on the battlefield and had level counters on it, so it no longer has any level counters on it.
The above answer applies to any type of counter. Unless it specifically states it does (i.e. Skullbriar, the Walking Grave), a permanent changing zones loses any counters it may have on it as part of it becoming a new object with no memory of its previous existence.

Slitherhead (and other Scavenge cards) don't contradict anything that was mentioned before. It sounds like you are possibly confused about how the Scavenge mechanic works. Would you mind elaborating on why you think there's a problem here? First off, here's the rule from the Comprehensive Rules that explains what happens to counters, +1/+1 or otherwise:
121.2. Counters on an object are not retained if that object moves from one zone to another. The counters are not “removed”; they simply cease to exist. See rule 400.7
This doesn't have anything to do with how Scavenge works, really. Though, if a creature has +1/+1 counters on it for any reason, those counters won't be retained when it goes to the graveyard or any other zone, as explained in the quoted rule. Skullbriar, the Walking Grave is a notable exception, but that's of course because his ability specifically allows it to keep its counters.
