We know that Teferi is the blue walker, but who are the four others? Here are my thoughts:
Red: Jaya Ballard. She just seems like the most obvious choice considering her near memetic status and incredible popularity.
Black: Leshrac. Honestly, I've always liked him more than the obviously insane Tevesh Szat. Also a good excuse to reprint his minion in the deck (No, it is not on the reserved list.
Green: Freyalise. Probably the most well-known green pre-mending walker.
White: Serra. Let's face it, not using her would be a flavor fail.
Now, the only problem I see with this is that at least three of them are dead (Leshrac, Freyalise, and Serra), and we aren't 100% on the last one (Jaya). Still it is fun to speculate baselessly.
Do we know for sure each deck will get a commander-planeswalker? I haven't seen that.
It was mentioned that several planeswalkers from magics past would get planeswalker cards in the upcoming commander product, and that they would be useable as commanders.

Good, that's actually a really nice concept and it would suck if it was only on one or two cards.
I really hope they don't bother too much with who's alive and instead go for iconic ones like Serra.
I'd be happy to get Windgrace in lieu of Jaya, as they're both excellent.
Also, Commodore Guff is clearly superior to Serra.
Serra has more history and greater popularity though, also, it would be awesome to have her ultimate give you an emblem that gave you a serra angel at the beginning of each turn. No, not just your turn. EACH TURN.

Teferi lost his spark, so it's already established they are ignoring the current status of the walkers. I agree with Krishnath's guesses, it has to be Serra, Teferi, Leshrac-or-Tevesh, Jaya and Freyalise.
Honestly, I'd have loved if the set was six decks, with five monocolor, and a five color deck, with the five color one containing Taysir. But alas. >.<
Edit: Oh, wow. Someone just mentioned that they'd like to see Ravi (Grandmother Sengir) as one of the planeswalkers in the commander decks...

I disagree. I loathe Urza with the burning passion of a million exploding suns. Also, he is a cold blooded murderer. So there is that.
That's to be expected from a man with Mishras killing machine as his avatar
I give you that Urza might be a little bit intense sometimes, but he stands for almost a decade worth of amgics storyline and is almost as iconic as that little black flower, so he would deserve his own card.
Hey, the Mak Fawa are considerably older than Mishra, heck, they predate Yawgmoth himself.
(There where living dragon engines present on the plane that would be later known as Phyrexia when Dyfed took Yawgmoth there).
As for Urza having his own card, he has several already, I think it is time for other pre-mending planeswalkers to shine.
I can't even come up with any other monogreen walkers other than Freyalise, so gotta go with her. Personally, I'd rather see guff than Serra just because guff was so darn lovably nuts.
It's also interesting to speculate on the other four Legendaries. I'd love to see Stitcher Geralf in U, but I think it's likely that each legend will cover a different block (which would also make a legendary werewolf rather unlikely). Who else have we had who is rather popular, but never got a card? I mean, except for Fblthp...
I came up with Azax-Azog, the Demon Thane, from New Phyrexia, but he would very likely be B too...
Oh, there's an entire thread in Storyline about that. Good source for ideas.
I'd like to see Feather show up, but she's Boros so not likely in the monocolored decks. Maybe next year we get enemy color pairs?

Well, someone recently linked a Twitter post by MaRo where he stated that PWs that "died on screen" will very likely never appear as cards. He mentioned three, and I remember two: Urza and SERRA.
So, perhaps Commodore Guff is more likely. One may wonder a bit how far back they will go with all this, though. Obviously a lot of people here remember the Nine Titans, but your typical newbie (who MIGHT remember Teferi because he was reprinted in FtV: Legends) will have no idea who Lord Windgrace or Freyalise is...
Lord Windgrace is a Panther Warrior planeswalker, apparently from Urborg. He was one of Urza's Titans, and a master of red magic. He can be seen on Planeswalker's Fury.
Freyalise was a half-elf (if I recall correctly) planeswalker who started out as a red aligned mage, but shifted to green. She was worshiped as a deity in Fyndhorn and Llanowar. She can be seen on Planeswalker's Favor. If I recall correctly, she survived the Invasion, but died during Time Spiral.

Gaea is/was a true deity, not a planeswalker. She represents life in Dominaria. The various Maro Sorcerers are her chosen avatars and bridge the gap between her and the living world.

Feroz is blue, and since we have Teferi, it's not going to happen (this time).
Taysir is all five colors, it is a major part of his character, but I agree, would love to see him as a card. He is also Canonically the only Planeswalker to be brought back after dead (And then he wound up murdered by Urza, but that's a different story).
Pre-Mending Karn would still be colorless.
Alternate Reality Memnarch could also be interesting, but would also be blue.
The Leviathan would be black, or black/blue. It was specifically described as a Demonic Leviathan after all.

Looking at the list of planswalkers here is what I think are the potential commanders if I missed anyone let me know
Black: Leshrac, Tevesh Szat, Ravi Singer. Geyadrone Dihada, Ravidal
My preference would be for Tevesh Szat because who dosn't want to see the old demon dragon octopus on a card. I feel Leshrac is more likely though. I doubt that Ravi singer or Geyadone would be chosen because of obscurity the same for Ravidal.
White: Serra, Commodore Gruff, Daria.
Serra seems the obvious choice and is actually my pick. The Commodore would be fun but I cant see them passing up the chance to finally give us a Serra card. I am not 100% that Daria was white but I think she was so I am including her even though I think she is a very small possibility.
Green: Freyalise, Kristina of the Wood.
My moneys on Freyalise she was a more recently active character and seems to be more popular then Kristina.

I thought Titania was an elf or human. She is certainly depicted as such on Titania's Song.

Nope she is a Maro-Sorcerer who looks very human. Check the wiki. No were dose it say all Maro-Sorcerers have to be made of wood or look like a pile of leaves and really you don't get a good picture of her on Titania's song. The Vanguard card gives you a better picture she has a dryad thing going on there and is coming up out of flowers.
As to Tevesh Szat to each there own I was talking about what the art would look like not necessarily how people feel about the character. As to his sanity you should remember that "...measures of sanity among planeswalkers are hard to come by."
While I am at it I thought of a possible blue Tawnos. Its a long shot but it would amuse me to have the commanders being Teferi and Tawnos.
