Underhanded Designs seems like an ideal card to abuse along with Panharmonicon, cheap artifacts, and blink effects or recursion. The puzzleknots especially seem ripe for this, with their ETB triggers being good alone, and downright insane when you have Panharmonicon and Underhanded Designs out. Combine with some fabricate cards to create lots of little guys for you and it seems like you could build a decent combo engine here. Does anyone think this could actually be a deck?
Underhanded Designs is an underwhelming design. On the other hand, I've got real actual money on Panharmonicon flourishing. In Standard alone, double energy counters for 187s would be a game-winner with Ætherworks Marvel. In Modern/Legacy, think about the Imprint mechanic. You can cast two Instant spells for two mana with Isochron Scepter- let your imagination go from there. Blink has had such a generous card pool printed in recent years that I definitely agree that there's synergy in it.
The issue with Underhanded Designs + Panharmonicon is that the Designs have a mana cost for their first ability. If you copy the trigger with Panharmonicon, you still need to pay the cost for each trigger separately.
So to make this work, you need to mainly play 1- and 0-cost Artifacts. Don't know about you, but in that case i prefer the Aetherflux Reservoir + 4x Bone Saw + Paradoxical Outcome combo.
Of course, Underhanded Designs might slot into that for some additional lifegain: while the lifeloss is not actually important (50 damage should basically always suffice), the lifegain might be if you have a slow start. Panharmonicon on the other hand is a 4-cost artifact that would not do enough in that deck, so it'd probably not be included.
In the end, it's still all pretty janky combo stuff, but it's a start at least.
And now I had a crazy idea: Underhanded Designs is triggered by ALL artifacts, right? So, what if you used it in a UBR Treasure deck? Treasure enters, tap, sack, pay 1, hit for 1. Could be a good slow Life grinder.

But I think you're onto something there. I think I'd try to use at least a WB base that gives you access to Hidden Stockpile which can seriously speed uo your clock because you'll at least be getting two activations of Designs per turn.
