It definitely seems like she's likely to be The Wanderer. A subtle clue, and maybe a stretch, is that they casually use the word "wander" when describing her travel. I feel like that word has enough weight in that context that they'd avoid it if it wasn't a subtle nod.
Couple little issues with the Azusa=Wanderer theory (fixable with some storytelling, admittedly):
1) She is human and therefore mortal. She could have sparked pre-mending, but she would have lost the planeswalker-associated immortality during the mending about 60 years ago.
2) The Wanderer is a white-centered character while Azusa is green-centered.

I adore the art of Minamo on Azusa’s kimono as well. That ice realm is so intriguing as well. Could Azusa have described the Heart of Frost and run afoul of the Yuki-onna? Or is she indeed the wanderer? Seems WOTC is playing up the mystery here. Since Michiko is also a viable possibility.

No, it was the fact they didn't have enough mono-white planeswalkers so they made Teyo and pulled Wanderer a from future storyline to use a sneak peak (same for Kasmina and Dav for blue and black). They consider Ajani for mono-white but GW has even less walkers so they left them there.

Azusa could have been a PW but not the Wanderer specifically. Or have glimpsed the Kami realm. I suspect WOTC are creating intrigue and mystery to shroud the Wanderer's identity by having Azusa be a PW of old, but Michiko is surely still the best candidate.
I wonder if the Mending awakened Michiko's spark? The fact her spark is so unstable may be because she is half of Kamigawa's Worldsoul. Her spark may keep attempting to return her to Kamigawa without her focusing to remain off-plane.

Whoever the Wanderer is, their spark is already weird in the way it works - there could easily be an aspect to it. Anyway, they've kept plenty of mortals from dying of old age already. Just another one for the pile. Putting an event that keeps them alive into their biography isn't that hard.

If her whole point is to wander, maybe she could have a spark that is so volatile that it can take her not only through multiverse, but also through time.
Whether or not it is the case, they seem to want to imply that she is the Wanderer with this bit of history. That said, they could very well just want to keep people guessing with some already speculated things and new possibilities.
I wonder if she is in fact the wanderer if this will be a saga that transforms into a planeswalker.
