That’s some odd wording, I wonder why they had to specify “permanent, spell, or card not on the battlefield”?
“If a black or red card would be put into a graveyard, exile it instead” covers most of those bases and is less wordy, but I guess they really wanted to deny death triggers on black and red tokens.

I've seen old wording use “from anywhere” as well. It looks like they want to stop these cards from every zone, including exile. This would stop Madness and Flashback cards, and those are largely in red and black. It'll also stop Rebound cards, Suspend cards, and Jump Start cards.
Dies this card single-handedly beat modern dredge? They can't remove it, it exiles every dredger except loam and salvage, stops conflagrate flashback, stops creeping chill, and prevents recursion of bloodghast, amalgam, and ox...that's basically dredges entire deck right there minus 4 copies of narcomoeba.

This card is another hate creature towards red and black: they are basically a walking Tormod's Crypt + Leyline of the Void for R&B.
I'm really suprised that this card isn't legendary.
It is a very strong addition to Mono W Aggro-Control (Unspoiler).
