This card may actually be cheap enough for consideration as a card draw engine in Mono-White EDH decks... After the traditional trio of Clamp/Top/Rack, this looks like the next cheapest reusable card draw engine that can come online as of turn 2.
How does it work with Madness? Since Madness forces you to discard into exile, it still works... Right?
Since a card with Madness goes into exile when discarded, this won't interact with them at all. Even if you don't cast it, since the card went to exile first, it would lose track of it.
This is because it exiles as a trigger, not as a replacement.

Nah, since players are supposed to be somewhat of an analogs of planeswalkers I don’t think accidentally opening up a portal to a random plane would be that hard to recover from. However I do agree that discarding another bag of holding should maybe have the effect of destroying the bag.
Jokes about D&D rules aside, I wonder if this might see some play in decks that are particularly vulnerable to targeted discard. Sure players can’t get their cards back until they get to 4 mana, but it’s a 1 mana investment that forces the opponent to find an answer if they don’t want all the cards they just stole out of your hand to come back later.
This also could be interesting in decks that use Lion’s Eye Diamond. Sure, LED doesn’t provide enough mana to activate and get your hand back, but 2 LEDs does allow you to do this.
I bet there’s also a way to break this using balance.

I am fairly certain you can't, as the new bag of holding is seen as a different entity than the one you had on the field previously, so it would not consider the exiled cards to be exiled by itself. Someone will probably drop by with all the proper rule references though.

that is also the feeling I had, I was debating putting this in instead of Library of Leng in my Nekusar EDH deck but my playgroup plays too much mass-bounce for it to be worth it if that's the case.
If other rulings for similar cards hold. (There were a couple of them, but card names elude me.) Cards exiled with bag of holding will be returned by another bag of holding since they were exiled by bag of holding. Don't think it's differentiated.
I think its the opposite, see Karn for an example of not needing continuous control. Other copies of Karn allow it due to silver counters, and if he was worded differently it would function like this.

Unless they changed the rules specifically for this card, no, a second Bag will not be able to get the cards back. It's like how a second Ashiok, Nightmare Weaver could not get creatures an earlier Ashiok exiled.
You can reanimate things in instand speed, since the trigger ability of exiling goes to stack while the card is in the grave. Could Grishoalbrand be interested as additional copies of Faithless Looting ?

This seems to imply that cards discarded to madness that arent cast can still be found by Bag of Holding and exiled.

Definitely seems very powerful for Commander, especially in mono-Red. It's very easy to turn this into a powerful Red card advantage engine with cards like Daretti, Scrap Savant, Reforge the Soul, Fateful Showdown, Faithless Looting, Gamble, Jaya Ballard and so on. It preventing opponent's discards and being able to loot itself is also fantastic.
As for Standard... well I doubt it, but maybe? It's cheap enough to stop Thought Erasure, but sadly it also gets reseted via Teferi, Time Raveler. Still want to brew some spicy jank with Heartwarming Redemption tho.

this set has a lot of weird subthemes, this discard one is going to be a lot of fun with Glint-Horn Buccaneer and stuff with Retrace or something like Seismic Assault or Greater Good or maybe to do REALLY stupid stuff with Bazaar of Baghdad in vintage.
