On the wiki, it is said that Serra got her name from a Dominarian goddess, thus placing her birthplane as Dominaria. However, her Vanguard card says she was named after a benevolent goddess, without specifing from which plane. In the Homelands comic she is a majoe character, but her background is unknown. Later, she will be introduced again as a planeswalker who created a personale plane and meets Urza. However, Urza never says from which plane Serra hails. He, instead says that "She saw great tragedy, as I did on Dominaria, and she made this place, this plane, as a memorial to what she'd lost." To me this has always implied she isn't from Dominaria, but another plane that was ravaged by something unknown.
Is stated in a source that Serra is Dominarian, or it is just another assumption made by the fandom?
I don't know of anything that specifically says Serra is from Dominaria, no.
There was an assumption that not all Serra Angels were from Serra Planeswalker, and thus the goddess had to be from Dominaria and Serra, too... but I can't find anything to actually support that.
Ok. If anybody cannot find a source, I belive it would be correct to put Serra's homeplane as unknown, maybe with a "speculation" on her Dominarian origins (that however seems to be implicitly denied by Urza).
Has Serra Angel and other angels ever been outside of Serra's Realm (be it created or brought)? If they are sighted on Dominaria, then perhaps we could assume that Serra has some ties to the plane?

IIRC, before Urza collapsed Serra's Realm into the powerstone he evacuated many of the plane's citizens to Dominaria. As a result even if Serra had never been to Dominaria, there would be Serra Angels there.
Serra has surely been on Dominaria around 3800 AR, where she died. On her resting place in the land of Sursi, it was built a Cathedral in her honor.
Even before, Serra Angels have been sighted on Dominaria, as far as before the Brothers' War.
Is it verified that Sursi is located on Dominaria? Pre-Time Spiral, my notes never indicated which plane Sursi, the Temple, or Brother Angus (who found her body) were located on.

After she abandoned Serra's Realm, well, first, Radiant went loony. (And yes, I always found it funny that "Radiant" is a possible translation of Lucifer.) But secondly, she went to Ulgothra. There, she stayed with Feroz, as the entire plane was under hisprotection. After Feroz's death, Serra left Ulgothra for Dominaria. She died there, and the Cathedral of Serra was built in her honor.
