In both cases the Hydra would be 6/6. Power and toughness setting is applied in layer 7b, counters are applied in layer 7d. So we'd apply layer 7b first, setting the power and toughness to 1/1. After that the counter would be applied, so there are 5 +1/+1 counters on it, so it would end up as a 6/6.
Just to note, with humility on the battlefield the hydra would still enter with 5 +1/+1 counters. Humility's effect would not be applied as the hydra is entering the battlefield. So the hydra would have the ability that sets up the replacement effect as it enters and puts the counters on the hydra as it's entering the battlefield.
