If Gemstone Caverns is in your opening hand and you're not playing first, you may begin the game with Gemstone Caverns in play with a luck counter on it.
You put it into play during your opponent's turn not during yours.

if you dont declare that you ar putting it into play before your opponent starts to play cards for his first turn then your opponent could say that it is too late to start with it in play.
Just a quick note (that admitedly is somewhat of a nitpick), the Gemstone Caverns is never actually played/put into play if you use the ability that lets you start with it in play. From the game's standpoint, the game simply starts with it there. The same is true for the Guildpact Leylines.
That said, the above are correct. The decision of whether or not to begin the game with the Caverns in play is made after you decide to keep your hand, but before the game starts.
Edit: @mastab3690: Actually, if you say you are keeping your hand and your opponent doesn't give you time to put the Gemstone Caverns into play, you may still do it. However, if you did have time (or if whether or not you had time was part of the argument), that would be left up to the descretion of a judge.
It would involve at least some investigation into what each player said and how much time actually elapsed between you deciding to keep your hand the the game starting as well as how long it was between the time the game started and when you decided you wanted to start the game with a Gemstone Caverns in play.
Also, this would create an extremely subjective situation that would require actually being there and conducting and investigation, so there is really no way to given any criteria for how long is "too long."

Krash how would a judge decide if you had enough time to do so its not like in every game someone is going to ask if you are ready to start the game.

A: (immediately) I'll play <land>.
In this case, B never really had a chance to drop the Caverns into play, since A started his turn posthaste. It's true that B should mention that he has something to do, since things almost never happen before the game actually begins, but A should also give him a chance. Situations like this are usually miscommunications or instances of one player rushing thru things. Magic is not a sprint. This would be a good chance for a judge to educate the players on the value of clear communication, and on making sure the opponent gets a chance to do things/respond.
Well, to me since you're starting second, it kind of means that the first player has already said he'll keep his hand. (all mulliganed until he's happy)
At this point, you can put down the caverns (which means you're keeping the hand obviously) and TADA, problem solved. So methinks you put down the caverns at the point where you declare you keep this hand, so that no quarrels or misunderstandings can take place.
What should happen is that after B says he'll keep, A says "I have one card / two cards / no cards to start in play", then B does the same, then they both reveal and put them into play.
That's probably not going to happen. So if either player has such cards, they should say so immediately after the second player says that he's good with his hand.
