I have a question regarding panharmonicon. I have Mirko, Obsessive Theorist out which has whenever I surveil put a +1/+1 counter on Mirko. Then I play Master of Death which says when master of death enters the battlefield surveil 2. Would Mirko get two +1/+1 counters for each trigger of master of death. So 4 +1/+1 counters in total?

Master of death's ability is triggered by him entering the battlefield, so panharmonicus causes it to trigger twice indeed. Mirko's ability is not triggered by a permanent entering the battlefield (surveiling is no such event), thus panharmonicus does nothing for it. It does not matter that the surveiling happened a short time after a permanent entering the battlefield.
Thus, in your situation, you will surveil 2 twice and put 2 counters on Mirko.
