Say Ratchet, Field Medic makes you gain life with its lifelink ability (so 2 life) and you convert it and return a Claws of Gix (tapped) with its ability. You sacrifice the claw to itself and Ratched, Rescue Racer converts back again, then the claw makes you gain 1 life, and the Ratched once again converts and gets the claw back.
Is the part of "This ability triggers only once each turn" still remembered by the converted card ? so you cant repeat this process over and over (for as long as you have mana to do so).
It's the same object, and the ability is defined by the same text, so I'd venture the intent is that it's the same ability (whatever that means). It won't trigger again.

When a permanent transforms and transforms back, are the abilities before it transforms the same "instances" as after it transforms back?
Indeed, according to the release notes, converting is another word for transforming for purposes of the game.
