If both players control Nine lives and one of them activates Ratchet bomb's ability with three counters on it, destroying all nonland permanents with mana value 3, does the game end up being a draw, or can the player pick the order in which they are destroyed, or is there some fixed order?

Nine Lives has a triggered ability at the end, there. Ratchet Bomb destroys them both at the same time - because when Magic says to do a thing to stuff, it's always presumed to be at the same time - but what matters is how these triggered abilities are handled and how they resolve.
Two triggered abilities can't resolve at the same time; they will resolve as given by their order on the Stack. If two triggered abilities trigger from the same event, then, at the time they would be put on the Stack, first the player whose turn it is puts the triggered abilities they control onto the Stack in an order they choose, then the other players do the same in turn order.
Because of this behavior, the player who will lose is the player whose turn it is not. That player loses, that player leaves the game, and the remaining player is declared the winner with their Nine Lives trigger on the Stack.

There's a fixed order in this case. When mutliple triggered abilities controlled by different players go on the stack at the same time, they do so in active player, non-active player order (APNAP for short), the active player being the one whose turn it is. The abilities controlled by the non-active player go on the stack last and they resolve first. So the player with the Ratchet Bomb will want to activate it during their own turn, so that the lose the game trigger of their opponent resolves first.

Yes. In that case, since it's your opponent's turn, your lose the game trigger will be placed on the stack above theirs. Your trigger resolves, you lose the game, and the opponent is the victor, the game is over without their trigger resolving.
