1.) I have Trinisphere, my opponent tries to play Chalice of the Void on 1. i know he pays 3 for my effect, but does he still have to play 1 & 1 to get the single counter?
2.) Same as above but with 2 counters, 3+2+2?
3.) Chalice is set to 1, I have Trinisphere in play. I cast Demonic Consultation with 2B. does Chalice counter or is the price outside the range of Chalice?
For any X spell the value for X is chosen, the cost is calculated from that and then as a last step Trinisphere's ability is applied if applicable. So if they choose 0 for chalice of the void they pay 3 mana and it enters with zero counters. If they choose X=1 the mana cost is 2 which is upped to 3 because of Trinishpere which is what they pay. If X=2 the mana cost would be 4 so unaffected by Trinisphere.
For your last question it ends up countered. Chalice counters things based on the CMC of the card which is always the value on the upper right corner of the card and this does not change. The only exception is that spells with an X in their mana cost treat X as whatever number was chosen when the spell is cast.

I have untapped Trinisphere in all situations.
