Samurai of the Pale Curtain's second ability applies only to "permanent[s]", which exist only on the battlefield (C.R. 110.1), meaning it applies only if a permanent would be put into a graveyard from the battlefield as opposed to from anywhere else, including a player's hand. Compare that ability with Rest in Peace's second ability, which applies to a card or token that would be put into a graveyard from anywhere.
(Note that it's not damage, by itself, that destroys creatures, but rather a state-based action that could do so [C.R. 120.5, 704.5g-h, 701.7a]. A better example for question 1 is Murder, rather than Lightning Bolt. See also this thread.)

Correct on both counts. Samurai of the Pale Curtain refers to "permanents" with no other qualifier, so it's only referring to things that exist as permanents on the battlefield, and not "permanent cards" like things in library or hand.
Something would go to the graveyard from the battlefield? Exiled.

If Samurai of the Pale Curtain and some other creatures are on the battlefield, then opponent cast wrath of god. All creatures including samurai are destroyed at the same time.. would samurai still exile everything that got destroyed by wrath of god?
Yes. The effect of Samurai exists while Samurai is a permanent and doesn't stop existing until it moves. If the death event would happen and the Pale Curtain effect exists, they get exiled. The event is replaced not after it occurs, but before.
In other words, the Pale Curtain can't 'be drawn' until the Samurai has been destroyed, but since everything must be destroyed at the same time, those creatures must have been destroyed while the Pale Curtain effect was still active, replacing their deaths with exile.

Is it the same thing if for example samurai blocks a Prized Amalgam. Both creatures die at the same time, so both are exiled?
Samurai of the Pale Curtain's second ability applies to a permanent (including Prized Amalgam) that would go to the graveyard, even if it would do so at the same time as Samurai of the Pale Curtain would. Both permanents will be exiled rather than going to the graveyard.
