Any source adds 1 counter
PIR adds 1 more
DS doubles it
PIR adds 1 counter (because of DS ability)
DS adds counters (because of PIR ability)
PIR adds counter (because of DD ability)
etc.... never ending cycle.
1) Yes, Pir applies. And you can do even better if you apply Pir first, because then the counter gets doubled by DS as well. So (X+1)*2 loylaty counters, where X is the normal starting loyalty of the planeswalker. Note, that neither DS nor Pir put counters on anything. The counter placement comes from the planeswalker (or other card), the two cards just modify the event to let it get more.
2) Yes, though again, it is proliferate placing all the counters. And apparently you already knew about the possibility of optimal application of effects.
3) Because replacement effects only get one chance to apply to an event. Once they are applied, they cannot be applied again. (Exception: the commander replacement effect to put the commander into the command zone).
I never thought point 1 works as you wrote So the best thing is to apply all "counter adders" like PIR or Oath of Gideon to my incoming planeswalkers and then (as the last thing) use DS
