For example, if a player has 3 Basic Land Mountains and 3 Basic Land Forests all untapped and taps 1 Mountain and 1 Forest to cast Ground Assault, how much damage would Ground Assault do to target creature? Four, or six?
In addition, are tapped creatures still under a players control?
My understanding of permanents that a player controls are permanents that have been on the battlefield since the beginning of that players turn, regardless of them being tapped are not. However, I am not sure and have been unable to find anything in black and white that says so.
In general, an effect that refers to "lands" a player controls or "creatures" a player controls, without more, doesn't care whether those permanents are tapped or untapped (just as an effect that refers to "lands" without more or "creatures" without more doesn't, as in Armageddon); compare Ground Assault with Mana Geyser or Pallimud. For example, if you control two untapped lands and two tapped lands, you control a total of four lands for the purposes of Ground Assault (review C.R. 110.5).
EDIT (Jul. 9, 2019): One rule was renumbered with Core Set 2020.
EDIT (Aug. 3, 2021): Edited.
Control is a relationship that exists between a player and an object (permanent or spell). It is independent of qualities of the object like characteristics or status.
Tapping a permanent does not intrinsically change its controller.
A permanent comes onto the battlefield controlled by exactly one player, and is always controlled by exactly one player. Lands are controlled by the player who played them, and permanents that were cast as spells are controlled by the controller of the spell.

Ok, I am going to highjack my own thread with another topic, synergy cards for Armageddon.
So, my understanding of Armageddon is that it would destroy all lands on the battlefield, including your own. Is there an enchantment, or artifact that has an ability like, "return you lands destroyed this turn to the battlefield"?
Asking for my better half that is playing red-white against my red-green.
That's outside the scope of this forum. Try asking in General or Casual, we'll be happy to help you out there. But here, we only deal with rules questions.
My understanding of permanents that a player controls are permanents that have been on the battlefield since the beginning of that players turn
You might be confusing the rule about control (110.2) with the rule about 'summoning sickness':
302.6. (...) A creature can’t attack unless it has been under its controller’s control continuously since their most recent turn began. This rule is informally called the “summoning sickness” rule.
Are tapped lands still lands a player controls?

Also, I guess I was supposed to use the card tag Ground Assault
