Situation: I cast entwined Tooth and Nail for Kiki-Jiki, Mirror Breaker and Worldspine Wurm. I tap and target Worldspine with Kiki and put a copy of Worldspine (with haste) onto the battlefield. At my end step I sac the Worldspine token, triggering it's first ability, putting 3 5/5 Wurms onto the battlefield. Upon sacing the Worldspine, it would go to my graveyard, if I am correct. Now, would then shuffle my graveyard into my library, even though there is no actual Worldspine in my graveyard?
its second ability will trigger, since the token can go to the graveyard or anywhere else like any other permanent (C.R. 111.6), and
its third ability will trigger since the token can even exist in the graveyard (even though for a brief moment) (see also C.R. 111.8).
After that, the token will disappear as a state-based action (C.R. 117.3b, 117.5, 704.5d), and that happens before you put both triggered abilities on the stack (C.R. 117.5). However, when the latter ability resolves, you only shuffle your library (normally, you shuffle only "it", that is, Worldspine Wurm, in your library, and you still shuffle your library even though Worldspine Wurm isn't there anymore in this case [C.R. 701.20c]) (for who a token's owner is, see C.R. 111.2).

No, because Worldspine Wurm doesn't shuffle the graveyard into your library upon dying. In case you wanted to ask whether you still have to shuffle the Worldspine Wurm token itself into the library... I'm pretty sure you don't because it ceases to exist upon being put into the graveyard.
Tokens generally make it to the graveyard when they are destroyed or sacrificed so they will trigger any abilities. So, even though the token ceases to exist shortly after going to the graveyard, it does make it to the graveyard so you will shuffle your library even though there is nothing to shuffle into it.
