Like lets say I have a Simic Initiate with one +1/+1 counter and a Winding Constrictor. Then I play a Grizzly Bears and choose to move the +1/+1 counter from my Simic Initiate does the bear become a 4/4?
I am asking because the wording of graft does not explicitly say "remove" and "place," although I would be surprised if it didn't.
Yeah, that works. These rules should help clear up the interaction:
121.5. If an effect says to “move” a counter, it means to take that counter from the object it’s currently on and put it onto a second object. If the first and second objects are the same object, nothing happens. If the first object has no counters, nothing happens; the second object doesn’t get a counter put on it. If the second object (or any possible second objects) is no longer in the correct zone when the effect would move the counter, nothing happens; a counter isn’t removed from the first object.
121.6. Some spells and abilities refer to counters being “placed” on an object. This refers to putting counters on that object while it’s on the battlefield and also to an object that’s given counters as it enters the battlefield.
Counters are put on a permanent not only in the usual way, but also by giving that permanent those counters as it enters the battlefield or by moving a counter from elsewhere onto it (C.R. 121.6; under C.R. 121.5, "moving" a counter includes "put[ting]" it on an object). In your scenario, therefore, an additional counter would be put on Grizzly Bears (C.R. 702.57a). (And even Simic Initiate would enter the battlefield under your control with two +1/+1 counters, rather than just one, if you already control Winding Constrictor.)
EDIT (May 3): Since Amonkhet, counters are now referred to as being "put" rather than "placed" on an object.
EDIT (May 28, 2018; May 31, 2018; Jan. 29, 2019): Improve wording.
EDIT (Apr. 3, 2019): Add rule citation.
The bear would receive two +1/+1 counters. When you move a counter from one object to another it means to remove the counter from the first and place the same type of counter on the second. So the Winding Constrictor's replacement effect would apply and the bears would have two counters on them.
