1. Yes. "(I)t deals..." refers to the creature that dealt (not: successfully dealt) combat damage and made that ability trigger, not necessarily Saskia itself.
2. If a creature has double strike, it will usually assign combat damage in two different combat damage steps. Saskia's ability will trigger each time that creature deals combat damage to a player. Therefore, in your scenario, that creature can deal 8 damage total in a single combat phase.
a. Only damage dealt by an attacking or blocking creature as the combat damage step begins counts as combat damage. Damage dealt when Saskia's triggered ability resolves is not combat damage, even if it's dealt during the combat phase. Thus, in this scenario, only 4 of the 8 damage will count as combat damage.
3. Saskia's triggered ability causes the creature it refers to to deal damage when it resolves, without interfering on how infect affects that damage. Assuming your opponent is the chosen player, then in general, after that ability resolves, your opponent will have gotten a total of four poison counters.
