Worst card ever is One With Nothing. A big part of Magic is card advantage, you don't want to go to top deck mode. By playing One With Nothing your are giving your opponent the win or at least the advantage.
How about: Rapid Fire: to truly appreciate the amazing suck of this card, you have ton consider that Legends packs were available for about 4 hours after they were released, and when you crack one of these in the rare spot and compare it to the Moat that the kid next to you got, it WAS the worst card ever.
Juju Bubble: While probably not the worst, it is damned close. Just think, its effect is so powerful and undercosted that it needed cumulative upkeep and the "you do nothing at all" clause as well.
Sorrow's Path has to take top honors, however. I must agree with the majority here. I have actually seen that card in play a few times, but never used. My opponent was not afraid of killing his own creatures, he had Chaoslaced it, and his critters were all pro-red. No, he just never, ever saw the amazingly rare situation occur where there were enough legal targets to actually tap it. In the distant past, you could atleast Gauntlets of Chaos the sucker to the opponent and tap it with Icy Manipulator, creating a pretty fair effect. But, get this, the text was errataed by Wizards for apparent POWER LEVEL CONCERNS. So now it just sits in play, having no gainful effect at all, ever. Man, that is just a beautiful, steaming pile-of-turd of a history.
This card is so bad it's not even funny anymore. A (rare!!) creature that is as strong as the (untapped!!!) Forest you sacrifice (!!!) when it comes into play.

For those shaking their old man fists at Sorrow's Path, Ravnica gave us Light of Sanction and Master Warcraft. I actually built a white deck revolving around these and switching creatures with the Path, Banding creatures and General Jarkeld. I wouldn't recommend such a pursuit for people who prefer to win game with some degree of frequency, but it was fun a couple of times. Sorrow's Path only sucked for ten years or so. But then so did Lion's Eye Diamond, and look where that is.
To make a tired point, the 'worst' Magic card ever is a more subjective concept than "What is your favorite color to play?" I could call Vodalian Soldiers the worst card ever and someone could protest that they have a Merfolk deck where that extra point of toughness is crucial. The Worst Card EVER could get a jumpstart from a new set, so then there's a rush to declare a new worst card ever and it gets a little silly.
Unless you're a tournament ace who can't cut ice with anything but the best, spend some time playing with cards that don't always get love.
While not necessarily the worst, my least favorite card has got to be Vindictive Mob. The cost is atrocious, the flavor isn't there, and saprolings weren't enough of a presence to even warrant printing him in Ravnica.
I hate everything about that card. I would hope it hasn't even reached Casual decks.
All the legends lands that give your correct color legendary creature "bands with other legends" are pretty worthless. Tolaria is actually worse than a basic island in any format, its rough being worse than a basic land. Apocalypse chime is pretty horrid as well.
Are we all forgetting that Hidden Path is actually worse than its white counterpart in that the mana required is slightly harder.
Spinal Parasite is the only creature that I know of that has a negative toughness (assuming you don't use sunburst). Eww.
Wood Elemental is infinitely, infinitely worse, though. Four mana. For a 0/0. Sacrifice UNTAPPED forests. No. I'd rather have a ground glass milkshake.
I am in search of a certified played Sorrow's Path for my Jank Card collection, though.
I'd say Scornful Egotist was up there as well, but I seem to recall some blue draw spell being used with it.

Scornful Egotist - A 1/1 for 8 mana. The morph ability doesn't make it much better.
Scourge has ALOT of cards where converted mana cost matters. In fact it was a relatively high pick in triple scourge UR draft decks (though you really had to be black in that format).

I'm with glurman on this one. I think Sorrow's Path cannot ever possibly be a good card. I mean, at least Wood Elemental swings.

I've killed someone with Sorrows Path before I knew about the errata on it.

except a 1/2 soldier creature could be useful in a Knight-Captain of Eos deck in extended or a soldier deck in general....
Squire's actually playable, though, and is only overcosted by 1 in contrast to decent cards.
There are a lot of useless cards from early M:tG that are really easy targets for "worst card ever". Chimney Imp also comes up a lot for some reason. Still, there are some cards that probably didn't see much play in any format...
I would play scapegoat in a heart beat in any meta where Wrath of God were a huge contender....
