I've been thinking about it for a while, and Voidmage Apprentice is the only card that can hard counter a spell with Split Second- morph as a special action, trigger goes on the stack, nothing else can because Split Second. It's been my pet card ever since in EDH. What else have you?
Some more wonky cards where they tried something: Panglacial Wurm Goblin Game (Still insane that this is not a silver-bordered card and potentially a tournament playable card, which has incredible obscure judge questions, if a bag of M&Ms counts as 1 object or multiple for each piece)

True, however Sundial only ends your turn. Obeka gives the active player the choice to end their turn, but Time Stop is a hard stop of whoever's turn it is. Subtle but meaningful differences to be sure.

For a number of years this had been an obsession of mine. Over time many unique abilities have been revisited, so finding the truly unique ones takes some serious digging. Mirror Gallery and Mirror Box are fairly unique-ish. Karn Liberated's ability to restart the game is unique.

I did say hard counter but you got me on two of those. More overcosted countermagic to add to my Commander decks!
I think Summary Dismissal counts as unique- of the very few spells or effects that can just exile spells from the stack, this might be the only one that doesn't either give the card back O-Ring style (Ashiok's Erasure, Spell Queller) or do it by ending the turn. It's also one of two that counters all abilities (Kadena's Silencer), again without ending the turn.
Lich's Mirror tries to restart it, also a unique one. Especially unique is the interaction with Donating it to an opponent and killing them with poison (infinite unbreakable loop, draws the game), as the Professor pointed out.
Speaking of, I don't know if it's still unique, but Leeches removes poison counters.
I would argue that Lich’s Mirror is only trying to reset your game, where as Karn Liberated’s last ability is restarting the entire game for everyone. Actually restarting the game. Thus making them two very different things.
If anything, Lich’s Mirror is a one sided Sway of the Stars type effect.
When it comes to “unique” things in MTG, the further the game continues the less truly unique effects there will be mainly because everything in Magic gets revisited at some point. WOTC will take a unique card or mechanic from an earlier set and make it the focus of a newer set.
I do believe however that Leeches is still the only card that directly removes poison counters from players.

Because there are only so many cards with Ingest , and it requires low-CMC creatures with Ingest going out first in order to put the cards into their Exile Zone in order to pull to use the ability , if you Min/Max a deck around Void Attendant , it self-assembles into the exact same deck every time .
It·s like putting a puzzle together . I haven·t had the deck assembled for awhile now , sed if you use four of each card ,
in order to make the deck actually work you have to use the same creatures with the same abilities .
There·s a tiny bit of wiggle room depending on whether you want to go all-creatures or include instants like Swarm Surge , however to have anything remotely functional you have to use the exact creatures of its cohort .
I believe Equinox is the only card in the game that can counter a spell based on what that spell *could* do, instead of what it does or did.
Floral Spuzzem isn't particularly unique - it just has a very early version of the "saboteur" ability - but the printed version has a unique wording - a mistake in the templating has the Floral Spuzzem itself making a decision. (Pair it with R&D's Secret Lair for extra confusion.)
Obsidian Fireheart has unique reminder text that is more flavorful than explanatory.

Ritual of Subdual may be the only card that makes land produce colorless
