I'm not talking about weak cards that you just think are useless or cards you don't like the art of. No, I'm talking about the cards that when you play against them, something in you just snaps. Or when see them, you can't help but mumble to yourself in rage. These can be cards that just simply reading about them gets to you or that are prevalent in your meta and you're just sick of it. Or it could even be just one component in some long-as-heck combo that just rubs you wrong.
EX: For me it's Freyalise, Llanowar's Fury. Why do I hate this card? Because my friend made a really good elf EDH deck with her as the general. Her abilities just annoy me. Make more elves that make more mana (because elves can't get enough mana, right?), destroy artifacts/enchantments (well, there goes using my artifact deck...) and draw cards for the # of green creatures (on top of making them?!). Barring her abilities, Freyalise is stupidly easy to get out in a mono-green elf deck. Mana is such a non-issue for elves that getting rid of her doesn't even feel worth it. On top of that, because she can spawn elves, she's rarely without a blocker. Her sheer utility and ability to protect herself combined with the play style of elves just induces so much frustration in me that it's not even funny!
Overall, Freyalise just allows for one the most broken races of MtG to work even faster and more efficiently. Man, just thinking about that makes me wanna strangle something.
It's my least favorite general to play against for a lot of reasons. I know there are generals that are worse and can win games much faster, but for some reason I just hate MW more than anybody else. It's probably because I feel like there's no way to effectively stop him; and there really isn't. See, if he were just UR then he would be powerful but fair. But when you add G to the mix it just becomes too much to deal with. With G you now have access to a powerful ramp package and the ability to destroy almost any answers your opponents might have for him.
But even if you ignore all that he's still a 7/5 beatstick that grants all your guys haste. He's also pointless to kill because then he'll just come back next turn or the turn after and restore all the board presence you just took the time to destroy. Oh, and your counterspells? Those are useless now, by the way. The only cards I've found that slow him down are MLD spells and cards like Meddling Mage, but both have their own set of problems. The former is hugely hated by most of the community and just causes headaches to everyone at the table, and the latter is extremely niche and pretty easy to bounce so they can cast him. And even after you waste all your resources to stopping the general itself, you still have the rest of their deck to fight against, and URG is no slouch in commander.
When you separate all these problems it's not so bad. But altogether it's just....ugh.
Funny, I never mind Freyalise. Compared to her, Azusa, Omnath, Titania, and Azuri all seem much faster to the kill... Freyalise is good, but almost polite by green Standards.
In EDH, it's gotta be Prophet of Kruphix. It just puts that player so far ahead it makes any game with a resolved Prophet archenemy, and every player's turn the player who has Prophet out's turn as well.
Deadeye Navigator... that guy has never done a single fair thing in all of magic's history. A close second would be Kiki-Jiki, for all the same reasons.
The hatred has gone away as the standard has faded into the past, but mono black vampires in any block Zendikar was involved in usually ran a few Malakir Bloodwitch. I ran Boros Landfall at the time and that stupid card...it just walled me every time. Out of Bolt range and casually immune to Journey to Nowhere and half of my creatures. Just to add insult to injury, it gained them some life and had flying to block my Kor Skyfishers. The rage.

Yeah, Freyalise can't really damage you and she's mainly just support. But the thing is that all those others you just named, he has them too in the deck. They may not be the general, but there still present with Freyalise giving them room to work. Just watching an Elf board go off is...annoying. Watching an Elf board go off even faster than normal...RAGE.
any card that lets an opponent search my library...I'm a little OCD, and the idea of someone else digging in my library just does not sit well with me. It doesn't make me mad, I just can't tolerate it. If one of those spells gets pointed at me, the game is over and I'll find someone else to play against.
Oh, activate Top, you say? Mind if I go make a sandwich while you fiddle with your library? Oh, you both have Tops in play? Call me when it's my turn again. I'm gonna go join a draft for something to do in the meantime.
The most aggravating part is that it's the one artifact that I want to destroy on sight out of sheer annoyance, even if it's strategically incorrect, but you can't kill the damn thing.

So much this. This card is so annoying to play against because it slows down the game disgustingly.

Onering's 4 simple steps that let you solve any problem with Magic's gameplay
Whether its blue players countering your spells, red players burning you out, or combo, if you have a problem with an aspect of Magic's gameplay, you can fix it!
Step 1: Identify the problem. What aspect of Magic don't you like? Step 2: Find out how others deal with the problem. How do players deal with this aspect of the game when they run into it? Step 3: Do what those players do. Step 4: No more problem. Bonus: You are now better at Magic. Enjoy those extra wins!

Agreed. Especially obnoxious in EDH, where it tends to be wielded by slower players in a format that can already be annoyingly slow.
Often in my early period where I was just learning many of my friends used oblivion ring and it's ilk. I'll never forget the multi player game I had where everyone was casting their orings and journey to nowheres etc etc on my stuff just to mess with me. Hate that card and all its friends. Sitting there in my opponent's field taunting me with my things that I can't have because where does UR get enchantment destruction?

Surprised at all the hate for Top. It's never been an issue. People know when to use it and understand that wasting time doing anything (not just Top) is bad.

That's an easy one. Sigarda, Host of Herons. 4 turn clock with Hexproof and Flying evasion and it hates on sac mechanics so even if you are packing Liliana, StP, Go For the Throat etc, there's nothing you can do short of having a sweeper ready. To me, the card epitomizes what's wrong with the design of modern creature spells.

I guess that happens but it's always the player who goes next that uptaps and hammers people with whatever they Wurm'd in. The decks that are built to be interactive and tight get hosed.
