if you bought one of the old harper prism magic novels there was a coupon in the back you could cut out and mail in to redeem for certain cards. each book had a different card assigned to it.

Nope, I bought those books back in the day but never had any of the cards. Never bothered to send in the coupon for some reason. I have no idea what ever ended up happening to my copies of the books either, probably got thrown out during a move or sold at a garage sale.

I actually have one of the books (my mom found it at a book sale a long time ago and bought it for me but I've never had theurge to read it) which still has the coupon to redeem for a random card. There was no way to get a Mana Crypt for sure as you had the same chance of getting that, Sewers of Estark, etc.
There is also an expiration date which was sometime in 1996 if I remember correctly.

I still have all of my books sitting in the closet. I bought all of them up until the first Brothers' War book. Once they started covering the established storyline I kind of lost interest.
As for Mana Crypt, you got it for buying Final Sacrifice, the final book in the "Greensleeves" trilogy:

I actually lucked into a Mana Crypt just by randomly picking a book to mail the rebate form from; I ended up selling it prior to its rise to a $50ish card (it was in reasonably terrible condition, as I recall). Picked up another one, though, so I'm not kicking myself now that I've started a promo collection!
Foreign languages were distributed the same way. I had some Magic books in French, and you could send the coupon in it and redeem for a card in your language. Somehow as a kid I never bothered to send mine in, though it probably was Sewers or something like that.
Years ago when I was active on the Magic Librarities forum, there were a few people there that had sealed bricks with 50 copies of Mana Crypt. I have no idea where they got the bricks from. I always assumed that they were left over after the book offer expired.
Seriously doubt it.

Some books may have specific cards but the one I have is the opposite where it lists cards you could get but no requests would be accepted.
