I'm quite irritated with myself at this point; I seem to have thrown away one of my precious little Card Pages (that's what I get for cleaning) and on it was quite an interesting concept for a portal type card... so I gave a shot at doing it again biased from memory; and wept-
:3mana:, :symtap:: Reveal the top card of your library, if it's a creature, artifact, or land put that card into play tapped and you lose life equal to half that cards converted mana cost rounded down. Otherwise remove the revealed card from the game.
Extremely less cool than the original I fear. Flavor is basically a portal that is used to transfer portions of an attack force quickly from one reality to another. Any-one care to help put some innovation into this? I know the original had something to do with the top card or your library and the removed zone... but that's about it.

I would first look at Temporal Aperture. Then I would add that if you reveal a c/a/l card, you can repeat the process for free. Obviously this card would cost a lot more.
:?mana:, :symtap:: Reveal cards from the top of your library until you reveal an artifact, creature, or land card, put that card into play tapped. Remove all other cards revealed this way from the game, and you lose 1 life for each of the removed cards.
,T: Reveal the top card of you library. If its a creature, land, or artifact card, put it into play tapped. If it isn't remove it from the game and you lose 2 life and untap ~.
4,t: Name an artifact, creature, or land card. Reveal cards from the top of your library until you reveal the named card and put it into play. Put all other cards revealed this way into your graveyard. You lose life equal to the amount of cards put into your graveyard this way.
