I'm assuming you what this to be a triggered ability; so start it with "When," not "if." And while this idea sounds simple, this ability opens up some huge rules nightmares.
First, the mechanics of a zone-change trigger is very complicated. In general, you should not make the trigger event include both <in-play> -> <any other zone> and <any zone except in-play> -> <any other zone>. The short explanation for why is that the triggered ability looks at a <in-play> -> <any other zone> change from viewpoint of before the event happens, and <any zone except in-play> -> <any other zone> from the viewpoint of after the event happens. Things get very confusing if you have to track the card through that. (AFAIK, only one real card does it, Compost, and it has some very strange rulings, and WotC has vowed to not do it again with an effect that looks at any characteristics the card may have.)
Second, not all effects that remove a card from the game remove it face up. See Shared Fate or Knowledge Vault, for example. If either of those two effects removed your Test of Beliefs, who would know that it was supposed to trigger? If it was Shared Fate, does the player have to tell others if he doesn't want to shuffle it back in the deck? The trigger condition should include "removed face up" so that hidden cards are not an issue.

You wrote it with two costs (not a cost with two parts, you have two colons.) It should either target the creature to be removed, or say "you control" if you want it as a cost. And if you can remove a creature with Faith this way (which I assume is the idea), it is overpowered.

You can give the spell an activated ability. There is nothing wrong with that (with Haunt, spells can have triggered abilities).
