Spell Ricochet UR
Instant
Counter target spell. Then copy that spell X amount of times, where X is the number of opponents you have. If an opponent counters Spell Ricochet they copy the first spell countered by it not named Spell Ricochet. Spell Ricochet deals damage to each opponent equal to the mana value of the spell that was copied.
Maybe add an extra mana so it can't be imprinted on Isochron Scepter? Thoughts? Can you imagine this thing resolving with a Hive Mind or Eye of the Storm on board? Maybe with Knowledge Pool and/or Possibility Storm for shenaniganery? Maybe that's going a bit TOO far. I could see this being stupid with Magecraft like making 3 Treasures off of Storm-Kiln Artist. Maybe in a Grixis deck with Professor Onyx to have each opponent lose 6 life and you gain 6 life If this is in a 4 player EDH / Commander Pod.

I get a strange feeling you think the card does something different than what is written. As is its about 4 to 6 mana too cheap for its effect of Counterspell, Parallectric Feedback, Fork+. I don't actually know what the text about it being countered is supposed to do so it may even be as much as 8 undercosted.
It's definitely a crazy card, both in effect and powerlevel. MWU laid out some possible mistakes that could significantly change the value of the card but the card is insanely strong regardless.
3) if an opponent counters spell ricochet, spell ricochet didn't counter anything, so that clause doesn't do anything. What was it intended to do?
It was actually meant to deal damage to the player countering Spell Ricochet equal to the mana value of the spell countering Spell Ricochet and whenever Spell Ricochet copies a spell it counters itself.

2) this can counter + copy permanents (works rules-wise afaik, they enter as tokens). Was this intended?

It was intended to also make token copies of permanents similar to Lithoform Engine.
It was actually meant to deal damage to the player countering Spell Ricochet equal to the mana value of the spell countering Spell Ricochet and whenever Spell Ricochet copies a spell it counters itself.
Spell Ricochet also deals damage whenever it copies a spell on the stack not just for when it gets countered by another player.
