Tainted Truth would be a nightmare to get balanced. Against an infect deck, it's more problematic than Treasure Cruise. If you're planning an environment where self-poison is common, it's likewise going to be too powerful and a severe challenge to balance.
Phyrexian Ascendancy seems... interesting. I think you should probably follow the alt-win standards with this and make the win an upkeep trigger rather than an immediate effect. It doesn't have the surprise factor of other alt-win enchantments, since you can't set it up fully before playing it, but I feel that it's just standard procedure.
Tainted Truth should poison you an amount, either 1 or 2. At 1 you can still reasonably cast all 4 copies to combo off, at 2 you are running into actual costs and risks to run this card.
I agree with Saneatali that sludge bomb could probably be instant. This of course can change based on how much poison there is in the set, especially with self poison.
Phyrexian Ascendancy, without seeing more self poison this is an awful card. With the only current method of reasonably using this being proliferate with the few self poison cards it would be easier to kill the opponent with poison than to win with this card.
Tainted truth -- kinda week. You'd have to be facing against a poison/infect deck for it to be worth playing. Outside of a environment where it is being pushed poison decks are rarer than hen's teeth.
Should probably give you a poison counter to make it a bit more playable in a non-poison environment, and make it a bit riskier in an mega poison envirnnment.
Ascendancy -- It makes poison unimaginably a bad strategy. Maro didn't want anti-poison cards. This is exponentially far more powerful than mere poison removal. It can never be printed in a set that pushes poison.
How about environments that don't push poison? The set has to be self-poisoning as a strategy. Self poisoning as a strategy usually means making getting poison counters as a drawback, but that approach makes self-poisoning too strong if the opponent is not a poison deck itself (ie there's no risk to losing due to poison, since you control all the sources).

Sludge Bomb actually seems rather reasonable as an instant to me. Fall of the Hammer plus Tainted Strike.
