There are no appearance fees for the HOF for Grand Prix. Somehow you got the facts wrong.

Where tis really relevant is where you have 2 undefeated players and a grouping of X-1 players larger than 6. Than the not all of X-1 players can draw and get in, but the 2 undefeated players could and if forced to play could potentially lose their spot due to Tie Breakers.
Incorrect. If the prescribed number of rounds are followed, any player with 1 loss will make the top 8. That is a fact - you are welcome to do the math yourself.
As a hint on how to do the math, the number of players to add an extra round changes after 226, not 256.

Without being present and interviewing all the players and making sure of the facts of the situation, I feel it inadvisable to suggest potential infractions that may apply to the OP's situation. (Especially if it is being specific about the OP's situation, and not simply a discussion of the policy itself.)

In the swiss, a player may recieve an USC penalty for asking for a concession a second time. If two or three players had to be pressured - one of them could have said no the first time asked. Which would fall under that definition of USC.
Rebound has the spell sitting in Exile, waiting for you to recast it at the beginning of your next turn. But you don't simply put the spells on the stack. You need to put the Rebound triggers on the stack.
Once a Rebound trigger starts to resolve, all of the other rebound triggers have to wait until after the spell that gets put onto the stack resolves.
In some bizarre Constructed format where Phage the Untouchable is actually playable, Alvin and Betty are playing a game.
Betty casts Living End. Alvin returns his Japanese textless alternate art Phage the Untouchable, the only creature card in his graveyard, to the battlefield, and both players miss the trigger. Betty ends her turn.
Alvin casts Plague Wind, killing Betty's creatures. He attacks and deals combat damage to Betty with Phage, and again both players miss the trigger. He ends his turn.
Betty draws, plays a land, and ends her turn.
During Alvin's next turn, Betty realizes, "Wait a minute, that's Phage?"
"...Judge!"
Is the game effectively a draw?
What if Betty was at 4 life and caught the error immediately after marking her life total down to 0?

There are two owners, and on Thursday, when they have FNMs, the good owner is the one there. I reckon I'll stop going there on non-Thursdays. Besides, they only support Standard, and I want to invest into other formats, so I'll start going to other stores on the weekend when I have time. Also, they have no selection. Last time I was there, they had 3 packs of sleeves, and 2 deckboxes. They don't have Shards, M10 orZendikar boosters, and they only open 1 box for singles per set, and the store owner(s) help themselves to whatever cards they want from the box. Gah.
Do they distribute FNM prizes on a night that is not Friday? This is a pretty bad offense to Wizards.
One suggestion for avoiding this penalty in the future would be to put your dividers in different sleeves. Then the extra lands couldn't possibly be construed as being a part of your deck.
This is bad advise. Often cards in different sleeves (or not sleeved at all) are part of the sideboard. A judge may rule that any cards in the box that are 'playable' constitute a mismatch / game loss.
So I had a friend who played a River of Tears during the Oakland Grad Prix. He forgot that it sees itself coming into play and produces B. Now he then suspends a Ancestral Vision, his opponet says ok and he ends his turn. His opponet then untaps, goes through his upkeep, draws a card, looks at his hand then calls "JUDGE!" and calls him on him illigally casting his Ancestral Visions.
No matter when the opponent calls for the judge, your friend is still going to get an infraction for spending the wrong mana on a card/cost.

Zendikar, which contains Day of Judgement is fully legal in Standard up until the second that M11 rotates out.
Very unlikely that Day will be reprinted - people were upset that Elvish Visionary was reprinted in M10 and legal from Shards.

You can't use the same Spasm to tap their perments that you use to tap yours. The mode on any copies are the same as the mode on the original.
