Phantom is fine. It's an Evolve enabler, sure, but it's also a 5 power blocker or attacker if need be. Zephyr Spirit was so bad because it never did anything; this will kill basically anything that decides to tangle with it and survive to be recast next turn.
I believe the turn 4 rule is also where wizards doesn't want any decks to constitantly go off before turn 4 for the sake of the format.

The corollary is "Competitive combo decks will consistently be able to go off on turn 4." Competitive players like to get to the point and win as quickly as possible.
On the actual subject I'd bet male based on the name, but Trostani shows they're willing to buck their own name sourcing every so often so it's not definitive. Every other Legendary woman in Ravnica that I can think of ends with an -a.

The card numbers given in the spoiler are #147 for Boborygmos Enraged, and #150 for Domri Rade. One would assume that between the two would be Boros Charm and Dimir Charm, at #148 and #149. However, today we got cards named Deathpact Angel and Call of the Nightwing, which would also have to fit between those two early spoilers. Something's not right.

Without the Comprehensive Rules, there is no definitive answer. Based on the reminder text, though, it's just a basic triggered ability. With three permanents with Extort, you'd get three triggers. You could choose to pay for any or all of them.

Felllix has the right of it when he points out that you have two different cards there, and both the mechanics and flavor would be vastly improved by splitting it out somehow.
Also, as designed, it's entirely too narrow to see play in basically any format. No Limited deck will want it since it requires the equipped creature to be Legendary and no Constructed deck will want it because it's way too conditional and slow.
It's not. They're bleeding "lose 1 life" to white in hopes that people won't see what's wrong with it because the ability has hybrid cost and people have no idea what hybrid means.

To be fair, they did establish a "precedent" for this: Suture Priest. If they decide that's less a "crazy Phyrexia bleed" thing and more a "white can get this when it's convenient" thing, well, it is their game.
