IMO not playing Incorrigible Youths is wrong. The card is the big Madness payoff (along with Temper and Alms). It has the opportunity to put some serious pressure on your opponent.
Refuel? How? Spending 2 cards to draw 2 cards is not a way to "refuel." Sure you could get a madness trigger off it, but if it is only in the deck to trigger madness there are better ways to do that.

You make some good points, but it's not easy to leave three mana open on this deck when Incorrigible youths can be relevant (by turn 4). I'll test it anyway by removing both copies of Tormenting voice and I'll let you know how it went.

The madness trigger is the reason to play it. Refuels our hand and triggers madness. If it were only one or the other, we prolly wouldnt consider running it.

If you're going to play Tormenting Voice, I recommend the midrange version of the deck, as it is actually too slow a discard outlet in the aggro deck and is awful to try and play around. I'm going to try something else in my list for it. Any suggestions?

The problem is that it doesn't refuel. You get to cast a Madness spell if you have one in your hand when you cast it, but if you don't have the Madness card, the mana to cast it or anything else you're spending two cards to draw two cards.
If you do manage to cast a Madness card that's a little bit better, but you're still about neutral on cards so it still isn't refueling.
Why aren't every madness list running Call the bloodline? I feel like it's one of the best discard outlets in the format.
I'll cut my mainboardTormenting Voices and check out this card. The one problem I see with it is that it takes a turn to set up, but still, instant speed discard and life-gaining vamps seems quite reasonable.
Wow, you went all kinds of mid range. While mine would fall somewhere between this and the all out aggro version, I like it. Only thing I would cut 2x GDD for 2 more spells, maybe lightning ax for big burn and madness enabler.. The problem I see if your playig mid range with no actual removal main deck. The burn in the deck won't be that good late game where you need to clear a path and finish the opp. Maybeput the commands side board and put Ult price main.
Play test the heck out of it tho and let us know what you find.
Some of my thought process for my card choices. I ad been playing Mardu Dragon-Thopters with great success and wanted to try and port it over after rotation. This biggest loss is in Crackling Doom. I wanted to keep the dragon theme and still think Draconic Roar is one of the better removal spells out there. With that I ended up going with a pair of Dragonlord Kolaghan. Also, my thought process for not running any unconditional removal was that I wanted to clear the early board with burn and shift gears and start slamming large hastey, evasive beaters. All of my creatures have some form of evasion and all my spells are recur-able with Goblin Dark-Dwellers as well I am able to get any creatures back with Kolaghan's Command. The hope is that Alms of the Vein and Kalitas, Traitor of Ghet can give me enough life gain to keep putting pressure on as the game goes longer. I did not want to go full on Madness but wanted to maximize damage, could go to the dome while be able to recast with GDD. I tooled the SB to be more specific in the ways I can deal with threats. I can not wait to sleeve this up and take it for a spin.
Aussie I would say Duress transgress the mind (I was leaning to transgress because of delirium and it can get rid of problematic creatures as well), but duress is a turn one play.
I guess no one thinks that first strike enchantment is a good idea?
I use it post sideboard and is really good. You can attack without the fear of trade your creatures and on turn 4 your turn 1 will probably have 4 power and take down your opponent creature.
Incorrigible Youths seems really mediocre to me. The perfect scenario is a turn 3 youths and after that it's just meh. Too much work to get it online and even when it's online a 4/3 body with no evasion is really meh imo. Ravenous Bloodseeker is also a very mediocre card. If you aren't playing Incorrigible Youths then you really should cut this junk altogether. Goldnight Castigator I think will be the go to finisher for this deck. It's essentially a 4/5 flying haste which is immediate impact. The fact that is has haste makes it better than the Dragon imo. Also the drawback isn't really an issue. You shouldn't care about taking damage with this type of deck. You're more worried about killing your opponent as fast as possible and this allows you to do that.
Here's what I would go with and most likely play at game day.
I think this is probably the fastest version of this deck. Olivia's Bloodsworn and Heir of Falkenrath are the perfect two drops for this deck imo. Both have evasion and enable other things i.e. give your other creatures haste (super important in this deck) and enable madness. Olivia acts as a great 3 drop lord and Castigator is your 4 drop finisher. Neonate and Asylum Visitor are also extremely important as they allow you to dig for more threats/burn as the game progresses. I feel like in this deck your madness cards should be 2cmc at the most and even that is probably pushing it. In order for Olivia to work you want to be casting creatures (your 1 to 2 drops) and then casting the madness cost of the card you discarded but with such a low curve this card has to be 2cmc at the most. In this type of deck you are rarely if ever going to have over 5 lands. so casting a 2 drop into Youths or something like that is totally unrealistic imo

turn 1 gorger, turn two blood seeker and turn three youths is pretty hard to stop...

bloodseeker is prolly the best vamp in the set.
How often is that going to happen? Imo Youths is only good on turn 3 with madness. Everywhere else it is really mediocre. A 4/3 body with no evasion really isn't that scary with so many eldrazi around. The other thing is the madness cost is 3cmc. That means that you have to play mediocre cards like Bloodseeker just to enable more free discard. Apart from Neonate (which nobody runs) and Heir (great card) there is no other good options for discard free of cost. Olivia requires creatures to enter the battlefield in order for you to discard a card. This means you have to cast at the very least a 1cmc vamp and hope you have 4 mana available to cast Youths for madness. The more likely scenario is casting a 2cmc vamp which means youre going to need 5 mana available. In a super fast aggro deck like this that just isn't the case a lot of the time. I also don't see what's so great about bloodseeker. Given my stance on the pretty mediocre madness cards I dont see why you would run this guy. A 3/1 body with no haste or evasion. Really average imo. You can't even discard multiple times cause his ability is +2/-2.
I think if you're going to play this deck your madness cards need to be either cheap (1-2cmc) or really really good (nothing stands out atm), or you have fast, evasive threats with cheap madness as an added bonus. That's why in my list I've gone for 15 fliers and 8 1cmc(madness) 3dmg spells.
When I first looked at the list I too thought bloodseeker and youths would be insane but the more I thought about it the less I liked it. I guess we'll have to see when the set releases but I really don't think it's going to happen
Getting Youths out for madness will happen pretty consistently if you build the deck with enough discard effects to make it consistent.
Blood seekers is good because it is a free way to get madness at instant speed. Being a 1/3 isn't irrelevant. Frostburn wasn't half bad being a 1/4 that could swap.

seeker trades or eats attacks from A TON of todays turn 1-4 drops. thats why hes so good. the trap is using the lords for this deck. they aren't that good. and youths isn't a great card...but its better than nearly all of the other vamps in SOI. 4/3 is bad, 4/3 haste on turn 3 almost every game is pretty good. 4pwr haste is great in a world of planeswalkers.
The biggest problem with Incorrigible Youths is that it is pretty embarrassing when not cast for its madness cost. If you are trying to use a 2-drop creature to cast it, there is a good chance that your opponent will just kill that and strand it in your hand. At that point you won't be able to get another enabler out and then cast it until turn 4, when it is much worse.

I agree that Bloodseeker is mediocre, but is exactly as mediocre as Aquamoeba was back in Oddyssey block, and it showed on every deck. He's just useful. He allows you to activate madness for free every turn, and what this mechanic is about. Call the bloodline or lightning axe you have to spend one mana, Tormenting voice is two mana and Olivia needs you to play a creature as you said. This deck must have several ways of activate madness for free, hence bloodseeker, Liliana, Heretical Healer (transformed) and Insolent neonate. Neonate is an unblockable 1/1 and an auto-replaceable, madness-enabler blocker when needed. Heir is pretty good, but it only allows madness once per copy. She's a solid beater when transformed but not a madness enabler.

I started magic back in the odyssey days and Bloodseeker is no Aquamoeba. Aquamoeba allowed you to discard multiple times in a turn. Also It was backed by flashback cards / wonder. Plus you could stack damage on the stack, so it was a virtual 3/3. There is no good repeatable discard outlets for zero mana in standard. In my opinion, Call the bloodline is our best discard outlet. For 1 mana it gives you creatures instant speed and a 1/1 vampire friend with lifelink. I don't like the new oliva. She is just bad. I think Liliana, Heretical Healer (transformed) could have a place in the 75.
Build the deck like an aggro deck, not a midrange one. The madness deck cannot go as big as other decks no matter which way you build it. It will fold to midrange decks in the midgame. So try to kill them before that happens. Prioritize 1 drops and cheap removal.
I hate Incorrigible Youths but it's still here for now. IMO Heir of Falkenrath and Fiery Temper are easily the best cards in the deck. Bloodseeker is a horrible card and you guys need to abandon it as fast as you can. Neonate may in fact have a place in the deck. As someone who has been playing with Relentless Dead a lot, menace is a deceptively powerful ability.
Your build seems pretty aggro, and I think Alms of the Vein is perfect for it. Sinister Concoction is another card to consider to help clear out a blocker.
