bolt - because its bolt.
thoughtseize - one of the gatekeeper cards of the format (along with inquisition). without it, the format would be pretty miserable.
path - prime removal
kiki-jiki - like it or not, its a pillar of the format.
liliana - probably better than jace in the modern environment.
half the best cards in the format are run in the GBx shell. plenty of discussion to be had there, but this isn't the place for it.

I'll add Lili. Thanks
Not many have mentioned Kiki Jiki but now that I think about it, it should probably get a top 5 spot. Most combo decks use it as their win con. It's kind of the same as Tendrils of Agony in Legacy.
Still don't really think Lili is top 5 but I don't want to get too caught up on that subject.
ETA: can't figure out how to add to the poll. Anyone know how?
1.) Lightning Bolt
I played a Junk deck last PTQ and I was missing something - a good removal card. Now Junk has a lot of viable removal in Modern: Path to Exile, Liliana, Abrupt Decay, Smother and yet none of it had the impact that Bolt does have. So I splashed red just to include it and I was happy with the result. (Playing Red in modern has also opened up a ton of options too.)
2.) Deathrite Shaman
I have probably won 50% of my matches because of this dude alone. Not only he is a lightning rod of a plethora of removal which also helps you in the long run, but if he goes unanswered he wreaks havoc by adding a clock or killing graveyard-based combo/synergy decks. And he ramps so easily in Modern.
3.) Thoughtseize/Inquisition of Kozilek
Having a way to strip your opponent's best early play is invaluable in Modern and there is nothing better than these two. I personally prefer IoK because of lifeloss but either has their strengths and weaknesses.
4.) Snapcaster Mage
Reusing your best spells has proven to be back-breaking vs. some match-ups. The fact that he is a walker adds to his bonus.
5.) Vendilion Clique (seriously, this dude is not on the list.)
Read above for Thoughtseize/IoK plan for protecting yourself against combo/strong threats. He has a big head and flash and can trade easily with attackers. It can even shape your hand if you're desperate enough. The only issue I have with Clique is that it costs 1UU but other than that it is an all star.
6.) Tarmogoyf (I'm only mentioning him because he was my 5th pick on the poll. No Vendilion Clique is crazy mad.)
Meh, big beater, small cost which makes him easily protected and therefore a scary inclusion in most decks.
Crap, forgot V Clique too...I apologize, I sort of hastily made the poll so I was bound to forget a couple cards. I really wish I knew how to add options...
Looks like the top 4 are separating a little bit. 5th spot is still close though.
I'll add Liliana and V Clique as soon as I can but I don't see them really challenging for a top 5 spot. Right now it seems to be between Thoughtseize and Snapcaster.

#1 : fetchlands and not close, they define the entire format far more than any other card on the list.
#2 : Lightning Bolt - it's a level above everything else by it's pure efficiency/power.
#3-6 : Deathrite Shaman, Path to Exile, Snapcaster Mage, Tarmogoyf
#7-10 : Dark Confidant, Thoughtseize + Inquisition of Kozilek, Cryptic Command (why it's not on the list?)
All of the top 10 are playable in multiple decks and many decks that aren't focused simply try to play as many as possible of the top cards of the format.

Common sense is the most fairly distributed thing in the world, for each one thinks he is so well-endowed with it that even those who are hardest to satisfy in all other matters are not in the habit of desiring more of it than they already have. - René Descartes
I think a card like lightning bolt is bad in quite a few match ups to make it a top 5 card.
I also think cranial plating should be in the discussion. Either the artifact lands or cranial plating were getting banned when they were starting to create this format.
So should vendilion clique, the card is too good not to at least deserve an honorable mention.
And how did remand make it there when cryptic command and spell snare didnt?
Not including Spellskite is quite clearly just wrong. It's actually in the top 5 most played cards in the format and one of the only things that can battle head to head with Lighting Bolt, the clear number 1.

According to MTGO Stats these are the most played cards in Modern:
Though that doesn't mean to say these are all the most format defining card, since it would be hard press to claim that on Relic of Progenitus or Stony Silence
