@Gypsyhummus - I am more or less pre-sideboarded against Affinity so it isn't a problem for me.
They can kill me a turn faster than I can kill them, but I have a lot of removal to slow them down. Since they have almost no removal, a Captivating Vampire can really take over and allows me to have a far superior late game, especially post board with the Nighthawks.

Compared to previous lists I have cut Dark Confidant, Aether Vial, Faithless Looting and the main deck discard.
Confidant is an amazing card, without a doubt. But, I can't do much with the human body and taking a turn off to draw more vampires and play them later isn't part of my game plan. It is good value but too slow.
Vial is good, but doesn't help cast Gatekeeper and requires less lands, meaning less landfall triggers for bloodghast.
Looting is good, but I don't have anything insanely powerful to draw into that makes up for the tempo loss.
Discard is good, but it is another tempo loss, in a lot of match ups I would always board it out and the non-creature combo decks are pretty uncommon/inconsistent.
The extra burn makes sure Noble/Lacerator are able to punch through and is a huge advantage in match ups that focus on the board like Pod, Jund and Affinity. In match ups where I need the discard, like UWR, Scapeshift and Tron, I compensate by having a very tight curve and burning them out game 1, then boarding the discard in afterwards.

I don't think you should cut any creatures because they are important to how your deck functions. The Duress and Sign in Blood should be boarded out against a hyper-aggressive deck like Affinity.

Sorin's Thirst and Feast of Blood are generally weak cards but if you are mono-black and need the sideboard cards specifically for Affinity I think they are good.
A friend told me that Extirpate would be better than Surgical Extraction against a decks like eggs due to the split second.
I think the split second is definitely good against eggs but eggs has been weakened a lot since the last banning.
Being able to cast Surgical for 0 mana means you don't need to slow yourself down and can keep a good clock on them. It is still very good against the t1 Griselbrand deck, Pod and it allows you to Thoughtseize them and then Surgical on the same turn 1.

If you have a Kalastria Highborn out, it is fine to attack with just Bloodghast.
Sometimes it is still correct to attack with everything and let something else die, just to push damage.
Bloodghast really shines against Jund/UWR. They have so many interchangeable answers, but they need specific answers for Bloodghast, otherwise it keeps coming back. Bloodghast is important to beat any deck that relies on removal for protection.
This is what I'm running at the moment. Once thoughtseize comes out in theros and hopefully becomes affordable I'll add some into the sideboard. Would substitute more fetchlands and a third liliana in the sideboard but they are a bit too expensive at the moment.
As for maindeck, cutting the discard seems to have greatly streamlined the deck. With the Captivating Vampires and enough creature fuel you can get really quick kills. I really like having Falkenrath Aristocrats since they represent an actual threat when topdecking and can survive boardwipes. Also has great synergy with Kalastria Highborn and Bloodghast. Feast of blood, though not as good as terminate, I feel is a nice touch to mitigate lifeloss when playing lacerators.
Suggestions? I don't feel 100% sure about the sideboard but I feel like the maindeck is about as good as it's going to get unless wizards makes a new vampire.

I like having 22 lands that can play Gatekeeper, kicked. For a 23rd land, you could try a Mutavault, so you don't get flooded.
I don't know the B/W token deck very well, but, instead of Olivia you could play Illness in the Ranks. In any kind of creature mirror I usually just try to clog the board with as many creatures as possible, rely on my Nighthawks and Stromkirk Captains to make sure I don't die, and then start grinding them down with Captivating Vampire or Kalastria Highborn + Bloodghast.

I haven't been playing modern lately, but I remember having a good clock and using the discard to snipe a pyroclasm was the best way too win.
Don't be afraid to over-commit to the board, they either have the pyroclasm or they don't, but their deck isn't designed to find it. If you ignore the pyroclasm, play your creatures and aim for a t4 kill, the match up felt OK to me. I didn't feel like a strong favorite but with my list I didn't feel like I needed to add sideboard cards for the specific matchup.
Falkenrath Aristocrat can be good in this match up too if you play it.
But, I could be off, I haven't been playing modern at all lately and some things may have changed.
So, I have not been playing modern because I have been playing just a **** load of Theros Standard, and, you can have some really broken lines of play with Nykthos in green ramp decks.
You can get good devotion with these vamp decks for sure, but I think it would be better used with Erebos. All the vamp decks are aggressive 1-4cmc that swarm out sideways instead of up. Nykthos is a ramp up card.

I still play it when I get a chance, but I have had a lot of work to do and have been focusing on GPs/PTQs.
I did try a build with Nykthos and Erebos, it was kind of fun but not super great. You could "kind of go off" but you weren't able to do it particularly fast. You couldn't do anything really cool until turns 4-5 which is kind of late. I tried including Aether Vial and Deathrite Shaman but it just wasn't doing anything insane that quickly.
The list I am playing at my next LGS event is as follows and is pretty fun.
Your 3 spot looks crowded with Captains, Nighthawks, and Captivatings. Highborn is essentially a 3 drop too, as is Gatekeeper out of sideboard.
Since you're already into red with good fixing, plus running a plentiful 24 lands, I think Falkenrath Aristocrat would be a great finisher here.
Or
With all those fetches you should have little trouble activating Vampire Nocturnus
Either one, but something as the 4 spot seems legit.
And I'm not sold on Faithless Looting here. Getting Bloodghasts into the GY is ok I guess, but really what are you digging for?

In a perfect world, I would make all my creatures have the same cmc so that I could play every creature off the top of my deck with my Aether Vial. If I wasn't playing Aether Vial, but was still playing 24 mana sources, I agree that Aristocrat or Nocturnus would be appropriate.
Deathrite Shaman is another card that makes 3-cmc a sweet spot.
I am not sold on Faithless Looting either, it is kind of dumb. I like it, it does some fun things, but it isn't very powerful or very good.
If I wasn't so focused on Legacy/Standard/Sealed I would post a better list, but I just haven't been playing Modern much.
Maybe the Looting should be a 4 drop to raise the power level, but if you are going to be playing Aether Vial I think you want 11 creatures that are 2cmc and 10 creatures that are 3cmc, at the minimum. Or like in the Merfolk decks, all 1-2 cmc creatures and no 3cmc creatures.
I have basically just been playing Looting for the same reason I have been playing Confidant, I just like drawing cards, LOL.

With the new unbannings there will a significant shift in the metagame. Will B/r vampires be slightly more viable with the unbannings?
Bitterblossom almost doesn't matter since it seems to want to sacrifice its creatures to win with things like Blood Artist. Now, I'm sure that it would be more likely to attack with Falkenrath Aristocrat, but overall, death triggers seem to be what they want to do.
I think Vampire's viability goes up with the bannings. Jund is definitely the hardest match up and without Deathrite to speed up their deck, exile Bloodghasts and block Stromkirk Noble, the match up gets significantly better.
Also if you think Faeries is going to be a big deal, play Cavern of Souls.
If Zoo becomes popular (and I think it will) playing Dark Confidant and Discard gets worse.

In a perfect world, I would make all my creatures have the same cmc so that I could play every creature off the top of my deck with my Aether Vial. If I wasn't playing Aether Vial, but was still playing 24 mana sources, I agree that Aristocrat or Nocturnus would be appropriate.
Deathrite Shaman is another card that makes 3-cmc a sweet spot.
I am not sold on Faithless Looting either, it is kind of dumb. I like it, it does some fun things, but it isn't very powerful or very good.
If I wasn't so focused on Legacy/Standard/Sealed I would post a better list, but I just haven't been playing Modern much.
Maybe the Looting should be a 4 drop to raise the power level, but if you are going to be playing Aether Vial I think you want 11 creatures that are 2cmc and 10 creatures that are 3cmc, at the minimum. Or like in the Merfolk decks, all 1-2 cmc creatures and no 3cmc creatures.
I have basically just been playing Looting for the same reason I have been playing Confidant, I just like drawing cards, LOL.
I'm planning to go back and play Magic again, and I want to continue using my B/R Vampire deck.
I'm planning on playing modern and I was wondering which cards I need to drop and which ones I can keep. I've been reading through the suggested cards in this thread though I still want to maintain my "identity" so to speak.

Also if you think Faeries is going to be a big deal, play Cavern of Souls.
If Zoo becomes popular (and I think it will) playing Dark Confidant and Discard gets worse.
