We've all done some stupid things, some of us dumber than others I'd wager.
I was playing with Merieke Ri Berit as my commander and only creature on the field. She had Lightning Greaves on and my opponent had Luminarch Ascension. Without thinking about it I attacked with Merieke to keep Luminarch Ascension from getting another counter. I did not think that through.
I was playing Alesha, Who Smiles at Death and my opponent had Defense of the Heart out. In my turn, I methodically cut down my creature count to two by sacrificing everything else for as much value as I could gain out of it and felt quite pleased with myself. I then played a couple of enchantments and artifacts. Before ending the turn, I thought to myself, so what am I gonna do with that three mana I have left? Oh well, might as well play my Commander.
He proceeded to wreck me. Horribly. While laughing.
I wanted to wrath the board because it got crowded over at Marchesas side. Luckily the was Imprisoned in the Moon.
I thought what to use my Qasali Pridemage on before it would die, and I'm not sure why I ended up targeting the Moon Prison. Sure, Marchesas got her whole board back at end of turn. So stupid...
I don't understand this.
EDIT: Never mind, I was thinking of the new Marchesa.
OT: I once died to a Rakdos Charm after creating an infinite creature combo. This has taught me to never choose "ten billion" as my number for infinite combo counts.
I once decked myself in an Ezuri, Renegade LEADER elf all tribal deck with an artifact that draws per amount of creature type on battlefield for 4 mana.
Then there was time I forgot Atarka, World Render does not actually have Double Strike passively so I accidentally got her killed blocking an angel token.
I spent an entire game doing nothing but playing Cackling Counterpart on my Eternal Witness. All I tried to do the entire game was see how many Eternal Witnesses I could have on the battlefield. I even tried it again after a boardwipe by Regrowthing my Eternal Witness.

As long as Marchesa was trapped in the moon, the board wipe would kill all her creatures permanently, because they didn't have marchesa's effect to bring them back (imprisoned in the moon turns off all abilities of the creature). but once pridemage killed the enchantment, her effect is active again. Which means she'll be killed by the board wipe, but also all her other creatures will come back as long as they have a +1/+1 counter (and herself too, if she has a counter on her from somewhere).

Oh, also... I once killed myself with a Horizon Canopy, totally mindless that I had 1 life at the time. I had Worship out, so I thought I was safe, and I had actually already used the mana from Canopy. It wasn't until right after my turn that a friend had noticed I had killed myself. My friends all said I could take it back, but I thought it was too funny and decided that fair is fair and that I was dead. (I was playing a durdly Angus MacKenzie deck with Divine Intervention out, so it's not like I was doing anything productive for the game anyway )
Choosing to trigger Sylvan Library while Notion Thief was already out. They draw 2, I lose 4 life. Feelsbadman
Even funnier was someone else cast Brainstorm at the end of that same turn! I wasn't in a place to tease since I just made that mistake, but you better believe we all had a good laugh.
Had an opponent flip Erayo, Soratami Ascendant early. The table went around a few turns doing nothing until I cast Obliterate thinking I was doing everyone a favor.

Stormed out at the end of a long game with Mind's Desire while playing Mizzix. I was aware that the only thing left in the deck that could kill the last player was Empty the Warrens, but (needlessly) got to a high enough storm count to deck myself. She passed the turn back, I lost on my draw step with 30-some goblins in play and no other nonlands on the board.

...except after playing Repercussion, I had enough devotion that Mogis was now a creature. You can guess what my life total was.

I was playing Yisan last night and I totally derped. For some reason when my deck was cut, there were about 20 cards lying to the side and I didn't notice it. I wanted to get craterhoof in play, but I couldn't find it. So I decided with my infinite mana to repeatedly cast acidic slime to kill the board. I love Sabertooth, then I noticed a stack of 20 cards which had my Craterhoof, thousand year elixer, and concordant crossroads........ derp
4 way game. Guy was playing something blue based, drops a jace flip walker...gets it killed almost instantly. I'm playing meren with a fairly full graveyard,but I have just topdecked persist faerie to steal it from his graveyard, tap, draw, discard, and then read the card to realize I just handed him back his painstakingly killed walker. Lesson of the day? RTFC! lol
One of the first few games I had with Animar I played Primal Surge while having Soul of the Harvest out and then FORGOT that Soul of the Harvest has a 'MAY' in it's effect and drew myself out. Even better, I didn't remember until I had already put everything in the deck for the next game.
Not one of my better moments, but if got a laugh from all of my friends and me.
The one that I tend to be reminded about although was a 1v1 match when my opponent EOT fact or fiction'ed. I even took the time to look over the 5 cards, since I was interested in possible reanimation targets. I was preparing to just mind twist his hand away on my next turn, and I didn't see anything immediately threatening, so I just gave him the friendly 5-0. Even with me visually seeing and holding his twincast in my hand for a moment from the FoF stack, I still went for the twist without even considering that it may be copied back in to my own face.
Was in a game against a Brago, King Eternal deck in which I had windmill slammed a Torpor Orb early on. I was having so much fun reminding him of the no-ETB-trigger effect every time he played one that I forgot it applied to everyone... then I played a Hoarding Dragon. >__<
In a game from earlier tonight, one of my opponents used Vraska the Unseen's [-7].
He then proceeded to forget he had the tokens before passing turn for multiple turns, when there were multiple opponents unable to block them.
That is one of the reasons I won that game
Once I paid a heap of mana into someone's Collective Voyage without seeing that there was a Zo-Zu the Punisher in play. The best part is, nobody noticed Zo-Zu until the lands had already entered play.
Opponent A: "Hey what's that?" *points to a card*
Opponent B: "Is that...?" *leans in to get a closer look* "Wait, <so and so> has Zo-Zu in play!
Me: "...oh."

I once thought Oblivion Sower's cast trigger was an ETB so I tried to clone it and all I got was a lousy creature.
One time I cast Rise of the Dark Realms without noticing that a Desolation Giant was in one of the graveyards.
