The actual list is too large to list, and reasons can change from meta to meta, or even deck to deck. So I may compile a list eentually, but for now, what are your most hated generals?
That's mainly it... the decks built around those generals --when all tuned and good-- can be devastating and boring at the same time. They all typically do one thing, so it turns into a game of "stop that deck from doing what it does," making it harder for me to do what I want to do.
for my meta, the deck is normally a bigger issue than the general. We all tend to play decks with the commander as a supplement, rather than the whole show, making the "What deck archetype do you KoS first" a more relevant thread for my meta.

I dunno. My friend is on reddit a lot and every thread he goes to Grand Arbiter is always listed as the most hated.
Riku of Two Reflections
----------------------------------------
Every single Riku deck is the same obnoxious thing. I love how Kaalia of the Vast gets the most heat of the commander deck generals for all decks being the same thing when I seriously have seen more variety in Kaalia decks than I do with Riku decks.
Sharuum the Hegemon
----------------------------------------
Now remember you would've never been crippled so badly by my Sadistic Sacrament if you ran more answers and less infinite combos you got from the internet.
Nekusar, the Mindrazer
----------------------------------------
I am glad all my decks accidentally have counters against him because it's been a long time since I played a game without someone playing Nekusar. It doesn't help that every single Nekusar deck is the exact same thing. It's also annoying to be constantly updating my life total every turn.
Oloro, Ageless Ascetic
----------------------------------------
Also pretty commonly used and it's annoying to be constantly keeping track of the Oloro player's life because you know the dude's gonna lie about the amount of life he gained and played.
Rafiq of the many
----------------------------------------
The Street Fighter 4 Ken of EDH.
Seton, Krosan Protector
----------------------------------------
Only because I have a very irrational hatred for Gilt-Leaf Archdruid which is like an auto-include in these decks.
Azusa, Lost but Seeking
----------------------------------------
It's more of a player issue than the deck. I've had to deal with so many bad players that just throw out an MLD spell "to deal with the ramping" when our Azusa player knows a hell of a lot better than to over-extend. Thanks for handing him the game you twits.
Yeah I know I'm kinda weird since my ire seems to be more focused on their players than the decks themselves.
The ones with built in Hexproof. At least Shroud effected the person playing the deck. Now I am forced to run Glaring Spotlight in decks that don't have enough sweep effects.
I don't want to run super narrow cards like Glaring Spotlight.

There is a thread two threads below this one about how Eye of Doom doesn't target.
Also, why would you ever not play wraths in a deck?

If combo should die before I wake I'll slide a Smokestack in every deck I play, roll in every shop wreck the Spirit of EDH.
Niv-Mizzet, the Firemind always kinda bugged me. The ability to set up 2 card combo's when one of the cards is your commander seemed like easy mode. Backed up with counter magic and even if you do have an answer they just tell you "No".
Saffi Eriksdotter was another one that ran rampant for awhile, until I realized what grave hate was. The way the deck combo'd out, regardless of what kill spells you held, was irritating to say the least.

Izzet is just a fun color combo regardless of what general you wun. be it Nin, Melek, Niv-mizzet or another. Izzet is FUN!
At the risk of sounding somewhat like a hypocrite, I don't like generals who are so linear they build the deck for you. Nekusar is a perfect example. Go to Gatherer, search for 'draw card' and 'discard', add land, and boom. Deck is built.
(I acknowledge that Animar and Karador want to be played a particular way to be most efficient, but even within that style, there is still plenty of variety.)

Pretty much my answer as well. I've gotten really tired of Nekusar really quickly.
1-Because the deck I was running was heavily creature based.
2-I am running colors without a lot of sweepers.
3-Doesn't fit theme of deck.
And yes Eye of Doom is another answer, albeit one that costs 6 manna to use.
1. Seems like you are too attached to your board state. A good healthy game of commander has multiple wraths as sometimes everything just needs to die.
Also, if you are unable to recover when the board is wrathed you are in trouble; if you don't play them other people will.
3. Making flavor based decisions is fine, but it is silly to complain about your opponents' strategies if you refuse to play perfectly reasonable answers to them.
6 mana is very reasonable for what Eye of Doom does, and should be fine unless you are playing ultra-competitively.

1- Depends on the deck, some are board dependent, most aren't.
2- Oblivion Ring can be played in any deck? (It also targets)
3- I am complaining about Hexproof, I am not complaining about my opponents utilizing Hexproof. Do you see the difference? I think putting Hexproof on a multitude of legendary creatures was a bad decision.
I didn't say 6 was unreasonable just it is slow, particularly if you have to tutor for it. I certainly didn't dismiss the card.

I disagree.
I feel every deck should have a way to wipe the board and be able to recover from it.
I was commenting about hexproof generals, rather than hexproof in general.
Building around a general with hexproof is just another strategy.
This thread is about generals people dislike, and what people dislike is really subjective.
It's perfectly fine to dislike hexproof generals, I just find your reasoning strange.

The EDH stax primer When you absolutely, positively got to kill every permanent in the room, accept no substitutes.
Azami - The deck have too much counter spells, combo and if you don't kill this general fast it will be a guaranteed lose (depend on the deck and player).
Mayael - I saw too much of this deck and every Mayael deck is the same.
Mimeoplasm - The general is strong and the deck plays best 3 color.
Edric - super fast takes I 15 turns by turn 4 plays lots of counters
5c - I don't like hermit Druid
Everything else I don't mind too much
@ phill that too playing against mutiple players all using the same style derevi deck is very lame indeed. I feel like wizards made some of the new commanders "to obvious" as to how to build them.
A quick list for me is
-Grand Arbiter Augustin IV
-Iona, Shield of Emeria
-Linvala, Keeper of Silence
-Sharuum the Hegemon
-Gaddock Teeg
so what is yours?

My playgroup has a different view of this, and they generally distrust stax generals, such as Hokori, Dust Drinker.
