EDIT: I'll 2nd Read the Bones. One of the best draw spells in black and I still don't see many playing it. Super underrated card.
Its like a way better Oversold Cemetery because you can do the recovery immediately and you activate it at will when you want.
I will also nominate Corpse Dance.
When you utilize it with some sacrifice outlets it can be incredibly difficult to stop. The only ways to really stop it are to counterspell, wheel hands, or force discard. Stripping graveyards only sets it back so its just a dang robust card.

I'd like to nominate Painful Quandry because it shuts down most combos and makes people question every card they cast.
I'd also like to nominate Chains of Mephistopheles as another combo stunter. While there aren't alot of artifacts that draw cards, every deck will play with what ever draw it can get. This card hurts you just as much, but if you play smart you can easily minimize your own lose

Concerning when returning to Kamigawa would be acceptable
As far as taking cards off, I think Ashes to Ashes deserves to come off, as removal has just gotten better over the years, such as Toxic Deluge. Promise of Power suffers from the same problem, along with being a huge mana and life investment. In the end, it's better to run the smaller versions of this effect, and let Necropotence and Phyrexian Arena do the "I need a large amount of cards for life" job. Sorin Markov strikes me as only being on the list for his -3 effect, and that effect never strikes me as particularly powerful. He's not a bad walker, just one that gets a lot of hype for something that's not that big a deal.
I'll second Phyrexian Reclamation.
I'll support Toxic Deluge. Cheap black board wipe that deals with the scourge that is Avacyn.

Toxic Deluge is just amazing.
EDIT: I'll use my other vote to nominate Night's Whisper. I think it's better than Read the Bones because it costs one less mana, which is a big deal in the early game. A lot of decks probably want to play both though.

Do you find yourself doing her +1 a lot? I am a bit curious as I have always disliked her when I have seen her played in multiplayer. I feel her edict is too narrow too often and I would have much preferred it to have been an edict for everyone. I think we have established that our metas are very different but I am just curious as to how she generally functions for you.
My experience has always been sort of you drop it and hit the voltron player with her -2 and then everyone else mobs her to death with tokens / etb guys.

Senconding Painful Quandary as it creates an interesting game state.
Also I am curious to know why Ashes to Ashes is getting that many votes. There is not a lot of removal that exiles in mono B. I consider this spell much more useful than others on the list.

You and me both.
When I first started playing EDH, I had two copies and they were shoo-ins to my B decks.
Since then, Sever the Bloodline, Gild, and Silence the Believers were printed, all with their own upsides. Exiling in mono-black is no longer so unique, and Ashes to Ashes is arguably no longer the best at it. In decks without a lot of life gain, I give Ashes to Ashes a second thought. Sever the Bloodline can take out token swarms, and has great utility in games where everyone is copying Avacyn. And Silence the Believers is an instant and in big mana decks (which most of my B decks tend to be), it can easily hit more targets.

For token swarms, I'd rather have Massacre Wurm, altough I could see Sever the Bloodline being useful if the tokens have more than 2 thoughness but that is quite rare.
With all that said I still think that 3 of the 4 nominations are superior to Ashes to Ashes so whatever happens, the list will be better.
