First of all, what do we all consider to be toxic gameplay?
Well, in my opinion toxic game play is anything that prevents people from playing their decks. For example, decks that revolve around making your deck not function like discard exile decks or things that drag the game out longer than need be with cards like thrives auction and such. But that's just my opinion and I have more examples but im writing this on my phone.
Some people feel that winning in 5 or less turns is toxic, but I don't feel ad though that's the same thing.
Tainted Strike is pretty toxic. But to be serious, people hate land destro, counters, combo, ect... and I hate tainted strike.
"Oh, you dont block this 10 dmg because you have 40 life? I'll just kick you out of the game even though i'm not involved in combat at all by spending 1 mana." I just think it's a pretty lame card in a multiplayer format. It's not even big and flashy.

there is one dude in my playgroup that has a skittles deck that literally runs almost every single one shot discard spell that he can (thoughtseize, duress, and the mass ones: Mind Sludge, Mind Twist, etc). sometimes it can feel "toxic" when he ramps into one of the X discards and hits me on 3rd turn or something, but most of the time, he'll go after my buddies Thrax deck because it has a few counterspells in it :D. it's actually turned out to be a pretty fun aspect of our playgroup, encouraging other people to run redirect effects and such, which always have hilarious potential.

Most of the time, this. There are a few times though, when complaining is kinda warranted, but there's always something more productive you can be doing.
Decks that shut down the game are fine, as long as they can win. If they can win, I can just concede and move on to a fun game. Otherwise, the deck doesn't really have a point, and if you find that fun, alright, but I really don't, so I'll go play with someone else.
I think it would be those lockdown style decks that just completely stop people from doing anything but drawing and passing the turn. Stuff like Winter Orb or Static Orb locks. Also large amounts of land destruction are pretty boring too, especially Crucible of Worlds and Strip Mine.
The "Vendetta" people who lose to you one game and the next few games focus only on you, despite game states that would have most players play differently. This is such poor form IMO and speaks volumes about how bad that type of player generally is.
People will complain about anything, and taking away a deck's ability to play isn't bad in and of itself. If your deck wins off beating with creatures and I Wrath you, you have no right to complain. If your deck wins off land ramping and I destroy all your lands, you shouldn't be complaining. Sure, it sucks either way, and it might mentally suck more getting your lands wiped, but it's a game and someone's out to win.
There are certainly cards that feel like a kick in the nuts, and those can arguably be called "toxic," things like Identity Crisis or Mind Twist or any other number of things that wipe one player out but not all (Wake of Destruction on the only mono green guy), but I think powerful cards like Myojin of Night's Reach aren't as bad because now everyone else will be trying to get back at that player, and I like that dynamic.
I'm generally not a fan of neutering one person and one person only, unless it's outright necessary to prevent them from winning, or from winning yourself.
Toxic? Players that take too long. If you're taking 5-10+ minutes per turn is bad. Taking that long to decide that you're going to do nothing is even worse.
Attitudes can be very toxic. It affects me way more than card choices.
Whining is pretty bad (that card isn't fair), smug elitism is worse (your strategy sux bro seriously), and apathy just kills it for me (oh... uh, my turn??) . I understand everyone plays the game differently and that's fine, but please at least be into the game.
