I can never quite get this card to play the way I want it to.
If I was so far ahead in card advantage, I usually wasn't very interested in effectively cycling the cards in my hand. If I'm behind, I usually don't want to cycle my opponents hand.

It's also fun in large multiplayer. Encourage people to muligan a bit, and play it first turn. The potential to screw up people's manabases is extremely high.
As vanilla creatures go, Fire Elemental is a good midgame threat in limited. It's big enough to matter when it comes down, and it usually can be played early enough to generate pressure or lift it. Green has tons of such creatures but red usually lacks the big stuff outside of rare/mythic.
Props to Melissa Benson... mmm fire boobies. doesn't compare to Rebecca Guay's "Shower scene" though... *ahem*
Back on topic, i agree with Rezz, red doesn't have many beefy vanillas.

Considering, what you can get nowadays for 5 mana in green, even just at common, this card hasn't aged well. These days, 4+ power with trample often comes with an additional upside, and usually with 4+ toughness, too. Nevertheless, 4/3 with trample for 5 mana would still playable filler in Limited, even with that drawback ability.
There are lots of cards in the Masques block similar to this one where your opponents spend mana on your cards. I'm working on an EDH deck that includes these cards then punishes my opponent for spending mana on my stuff.

imo, it's not that the card hasn't aged well, it's more that newer players haven't seen the power creep just kick this game in the nuts- Eldrazi, Mythics, & Planeswalkers, et al.
A quick run though of similar cards, there are "three" 4/3 for 4 vanilla creatures out there- Broodhunter Wurm, Nettle Swin, & Order of the Sacred Bell. The Elephant isn't bad, of course there are "better" cards out there.
The biggest problem in the card, was the set itself. Prophecy was a mess of a set, with a good half-dozen mechanics running around- Rhystics, "tapped out" card, 'ransomed' creatures...

The Elephant is just far below today's standards, and will keep "getting worse" as more and more new cards outshine it. But a 4/3 for a reasonable cost and a mild drawback should always be good enough to see some play in Limited.

Either they let you abuse Chain and Scandalmonger to empty their hand without penalty, which fuels 8Rack and Nyxathid, or else they risk getting burned out by Psychic Purge and Guerilla Tactics.
Wrath of God combos with Indestructible-granting effects.
One cute combo is to attack with Frontline Medic and 2 other creatures, then cast Wrath postcombat. Who said aggro decks can't Wrath?
You can also use Odric, Lunarch Marshal + any indestructible creature (e.g. Heliod, God of the Sun) to give your whole team indestructible. You don't even need to attack. Postcombat cast Wrath.

I used to have a deck with Scandalmonger and Chain of Smog that punishes the opponent through Dodecapod and Sand Golem. Unfortunately, the surprise only works once.

The most common combo is T: Win the game.
Commonly used in Commander and the reason for its banning in Legacy.
You could make a hilarious deck with:
