Can someone please clarify on first two triggered abilities triggering at once, giving you two times two knights? Rules say
714.2b “{rN} — [Effect]” means “When one or more lore counters are put onto this Saga, if the number of lore counters on it was less than N and became at least N, [effect].”
and this made me go and do a search for a rule which says counters are placed one at a time (like card draws), but I wasn't able to get one from there. Then, "or more" part made my concerns even stronger (like why it would even be there if counters are placed one at a time). So now I'm in believe that counters are placed in numbers at once. Saga had 0 then it had 2. 0 is less than 2 - so get your (II) effect and thats it.
Then I realized that each Saga just have multiple triggers which will trigger separately. Case closed

"directly" is "immediately" if you want to name it this way. In this scenario, Krovikan Horror will not trigger
PS. you can try some take-out-target-card-from-middle-of-graveyard cards like Deathrite Shaman (to take out swamp in your example and make Horror happy)

An activated ability is a mana ability if it meets three criteria: it doesn’t have a target, it could put mana into a player’s mana pool when it resolves, and it’s not a loyalty ability.
Both permanents mentioned above do have mana abilities, opponent will not be able to break mana-generating process (have no priority to do so)

Endless Whispers must be on the battlefield for it to trigger when Phage dies, after that point its existence on the battlefield changes nothing: your opponent will get his Phage surprise at end of turn.
He should consider Erasing Endless Whispers before destroying Phage

Exactly. "Up to one" pretty much includes zero, so there you can activate it just to get loyalty. For some reason (maybe Extort ability of other permanent you have) you can cast Launch the Fleet without targeting anything(choosing to make zero targets for "any number of target..."), but in case of Venser, the Sojourner, alas, you have to choose exactly one legal target
