Most of the ones I'd suggest have already been posted, but Vanilla Sky is one of that usually ends up being a mindscrew for most people I introduce it to. Tom Cruise plays Tom Cruise, as usual, but if you can put that aside and love all the horrible things that happen to him instead of loathe him (well, if you're one of the Tom Cruise loathing club members - I'm not, personally, but many people I know are), then it's fun to figure out exactly what the hell is going on.
Tokyo Gore Police is a Japanese Noir Film that makes so little since it's mind boggling. It's supposed to be extremely stupid, but the way the scenes are glued together and some of the character concepts are just a huge "WTF?".

The eternal sunshine of a spotless mind
Liked this movie far more than I expected to. It also gets props for being one of the coolest movie titles of all time
If we're gonna include anime as well, then FLCL and Serial Experiments Lain definitely make the cut. Still can't figure out wtf I was watching.

I just found Carrey a little funny and parts of the film poorly done.
Aronofsky's Pi was a mind-mess. I found it deeply irritating.
Just watched Oldboy, a foreign film at a friend's apartment. It's a 2003 Korean adaptation of the Japanese manga of the same name. Although it seems completely convoluted at first and proceeds with a slow pace, the build up in momentum after the story really picks up just unleashes a barrage of psychological suspense that will keep you captivated till the controversial ending. Being a little psychologically unhinged myself, I enjoyed the movie immensely both for its brief action sequences and artistic value. I fully recommend it to anyone who's truly looking for a film that "messes with your head." It's also based on a Greek tragedy, if that tells you anything, but I won't say which one as it might give away the plot.
Primer is a low budget time travelling movie that has gained a certain cult following. It's rather confusing, and here's a quite funny image, and that made me want to see it in the first place:

I don't think I would categorize Event Horizon as a "mess with your mind" movie, per say.
However, I absolutely love the underrated movie. I still have my VHS copy.

The problem is there are movies that mess with your mind because they are saying something coherent, but it is very complex — but there are also movies that mess with your mind because they are convoluted without ultimately saying anything that makes any sense.
Primer is an example of the latter. Primer is actually one of the oddest and most ironic examples of this, because it's a time travel movie where the part that's confusing, baffling, and ultimately doesn't make any sense at all isn't the time travel. It's the characters, and why they do what they're doing in the first place, and why I should ultimately care.
I may be a jerk for saying this, but I love "A Scanner Darkly" by Richard Linklater. I think it was only slightly screwy because of the way the information was presented. It actually made more sense as a book, which is weird seeing as it is one of Philip K. Dick's least confusing novels.

Too much CGI? Are you thinking of Life of Pi maybe? Pi is a black-and-white, low budget sci-fi film. And quite a good one.
