That performance preceded Marilyn Manson by a year ('89), it was made during the rise of grunge but before Nirvana or Pearl Jam really took off(I could be wrong).
So what do you think started "Goth", I'm putting my money on Wynona Ryder.

What do I think started "goth"? The simultaneously conflicting teenage desires to express how different and unique they are and to be part of a uniform group.

No doubt that is what "goth" turned into.
A: "I'm expressing my individuality!"
B: "But you look like all the other goth kids"
Surely that's not how it started, or who started it.
I used to think it was a cross between Green Days Billy Joe Armstrong during the Dookie rise, mixed with some Marilyn Manson.
But seeing the timeline, it occurred to me that Beetlejuice came first, and Lydia (Ryder) was spot on "Goth".

Goth goes waaayyy further back in time to bands like Siouxie and the Banshees, Rocket from the Crypt, Sisters of Mercy, Joy Division, Bauhaus, The Damned, Southern Death Cult, Mission UK, Alien Sex Fiend, and some synth pop like Depeche Mode and The Cure.
Goth took the styles of BDSM, Victorian, Elizabethan, early industrial (Throbbing Gristle, Skinny Puppy, Ministry, etc...), and punk. Goth and splatterpunk fiction (Clive Barker being the most prevalent/popular) and art (H.R. Giger comes to mind here), blended w/ the German art movements (such as found in the old Bauhaus and the New American Bauhaus in Chicago) and Art Nouveau.
Wynona Rider's Lydia just ran w/ the poppy after-thought that the Goth lifestyle/music scene left in the mouth of the public. Marilyn Manson appeals to the Goth audience, but himself isn't Goth. His music ranges from industrial rock to metal to hard rock to glam rock.
Goth started in the late 70's as a life style, sound, and form of self expression. Goth's roots run even earlier than that with Alice Cooper, Black Sabbath, David Bowie, Deep Purple, Blue Oyster Cult, T Rex, Iron Maiden, and oddly, The Velvet Underground/Lou Reed.
The way Wynona portrayed Lydia in the movie, which came out in 1988, (cartoon tv program ran from 89-91) could it be that she created "Goth"?
The way she rebelled against her rich haute parents with her special brand of depression.
