Proper procedure is Game Loss followed by correct the deck (remove all the illegal cards, and you can only add in Basic Lands if you need to bring it back up to 60)
FNM is "Regular" Rules Enforcement Level, so the only valid penalties a Judge can apply are either "fix it as best he can and explain to everyone what's going on" or "DQ someone for Cheating or serious Unsporting Conduct problems".
If the player knew Grave Titan was illegal but played it anyway, hoping no one would notice, then that's Cheating.
Otherwise, the Head Judge should replace the offending card with a basic land and remind the player to pay attention to these things when building a deck.
For a player who inadvertently brought a deck so full of illegal cards that there's no clean way to apply the "basic land" fix, the HJ might go for some other possible solutions as he sees fit (drop and "pair" with the Bye player each round, keep and have the player enter all match results as 0-2 regardless of actual results, etc.)
At an FNM you call a judge and they explain it's an illegal card and swap it for a basic land of their choice. Game losses at FNM are pretty much reserved for outright cheating, or behaving in an unbecoming fashion.
To be fair, it seems like this would have to be an honest case of not understanding how rotation works. I mean... would anyone REALLY think they could play an extremely popular (at least it used to be when it was legal), powerful card that's not legal in standard at FNM without anybody noticing?

I only used that as an example, this was for a post rotation question, but I thought I was going to get infracted for it, so I just used that. So I got my answer, and what to expect if I see it happen come standard rotation.
When I was still a judge and organizing FNMs, I tended to use the FNMs as a learning/teaching environment. Now, if one of the regulars came in and did something, I would be harsher. A lot of newer players don't realize how/when cards rotate.
Player A and B are playing standard, and player A has won the game they procede to sideboard, and Player B plays an gravetitan. What happens?
