Well first, I am not sure if we can answer cards that are unreleased, if not please disregard.
Black sun for 5 means that all creatures get 5 -1/-1 counters. When state based actions are checked, the creature dies. Now in your example it already had a +1/+1 counter. All state based actions occur at the same time, the creature goes to the graveyard, and the +1/+1 counter is removed with a -1/-1 counter. Since the creature has a +1/+1 counter when it died it stays dead, as the counter is seen as the creature goes to the graveyard.

Well, my understanding is that if it has a +1/+1 counter on it, regardless of whether or not it returned from the GY with it or was given it, it would still be sent the graveyard and NOT return (due to it being killed with a +1/+1 counter on it).
State based effects would have it killed because it would still see the +1/+1 counter on it at the time it died.
I thought that +1 and -1 counters don't exist on creatures simultaneously, they just cancel each other out?

What about if the -1/-1 counters don't kill it. Say my Strangleroot Geist comes back from undying with a +1/+1 counter on it. I cast Black Sun's Zenith for 1, leaving it as a 2/1. Next turn it get's hit with Gut Shot. Now does it come back through undying a second time?

Of course; the +1/+1 and -1/-1 have already cancelled each other out, meaning it has no counters on it.
