I've been playtesting a bit lately, primarily using the current format's MVP's like Thragtusk and Sphinx's Revelation, etc. and shoving them into a variety of different decks.
But it got me to thinking... Do you think there are card synergies and combinations that the creators intended, or at least recognized, that the MTG community hasn't been using or simply hasn't discovered?
On a similar note, how often are there synergies and combos the community has put in play that the creators didn't expect? (I would thing this is how bannings are born, but wonder how often it happens?)

its all but impossible for combos to exist in standard which haven't been discovered. there are many which are known to less than 50% of the player base, sure, but at least someone out there knows about every possible combo/synergy that exists in std. in eternal formats, there are probably undiscovered combos but they are very unlikely to be format breaking.
Every time I play Dredge in Legacy, I can't help but think that it was intentionally created by a few key WotC design and development employees over the years.

For sure, and there are always cards that R&D have set to be pushed as staple cards in a set that end up being nothing in the end. They've admited as much in the past with cards like Serra Avenger, who they were worried was going to be too strong at the time.
Also, look at promo cards for Game Days and release promos... these are cards that R&D thinks will be possible hits in Standard. They don't always hit right away, but some cards like Black Sun's Zenith and Emeria Angel did after rotation. Others are popular choices right away like Restoration Angel and Strangleroot Giest. Then you have some guesses that never quite stick, but you can see why they were chosen as possible contenders, like Dreadbridge Goliath and Killing Wave.
